Location: Southwick to Westfield

Responsibilities:

  • Assess the needs of home health care patients and families, considering physical, psychosocial, and environmental factors.

  • Develop, implement, and evaluate individualized plans of treatment.

  • Conduct initial evaluations and re-evaluations during each visit, adjusting care plans as necessary.

  • Communicate significant findings and changes in patient conditions to the clinical team and other relevant personnel.

  • Provide counseling, instruction, and support to patients, families, and caregivers.

  • Educate staff in alternate care settings on home health philosophy, pain management, and symptom management.

  • Supervise and document the delivery of care by home health aides and LPNs.

  • Handle patient property with care and maintain patient and staff privacy in accordance with HIPAA regulations.

  • Promote Elara Caring's philosophy, mission statement, and administrative policies.

Requirements:

  • Current, unrestricted state RN license.

  • Reliable vehicle, valid driver's license, and current auto insurance.

  • Preferred: Associate's or Bachelor's Degree in Nursing.

  • Preferred: Previous experience in home health or hospice care.
