GENERAL DESCRIPTION:
Performs nursing assessment, diagnosis, treatment, planning, implementation, and evaluation to individuals, families, and groups, or the population as a whole by providing services in the Maternal Child Health Program, the Immunization Program, the Chronic Disease Program, the Communicable Disease Program, and in general community health assessment and planning. Planning and preparing within the Public Health Emergency Preparedness program will also be part of the services provided.
This position is eligible for a $2,000 hiring bonus

ESSENTIAL FUNCTIONS: The listed functions are illustrative only and are not intended to describe every function that may be performed at the job level.
Provides direct nursing care in the clinic, home, and other settings, through the use of the nursing process, including teaching and counseling patient and family in patient’s care.
Documents nursing care given, according to State Standards for Documentation and promptly.
Plans and conducts health clinics and classes at various sites within the county.
Refers patients to other community resources and collaborates with those providers in carrying out patient care.
Represent the agency on local/county/state/national boards, committees, or organizations.
Work with the county nurse manager, agency staff, and state PH Emergency Preparedness team for the planning, training and implementation of public health emergency response.

KNOWLEDGE:
Knowledge of: Laws, principles, and practices of professional nursing; maintenance of health records and charts, reports, and observations; principles of nursing process and scientific process; public healthcare functions and essential services; applying nursing diagnosis and treatment to patient care; scientific concepts as a basis for decision making; physical, biological and behavioral sciences; evaluation of services and programs according to standards; work management and leadership theory and practice; state and local law on healthcare.
Skills in: Monitoring the work of others; applying the nursing process; interpersonal relationships; oral and written communication skills; computer familiarity and skills.

MINIMUM QUALIFICATIONS:
Education:
Associate's Degree (typically in Nursing)
Experience:
0-2 years of progressive work experience (typically in Nursing) with acquired knowledge at the level of an LPN
Certificates, Licenses, Registrations:
Must be licensed or eligible for licensure as a Registered Nurse (RN) in the State of Wyoming
OR
Education & Experience Substitution:
3-5 years of progressive work experience (typically in Nursing) with acquired knowledge at the level of an LPN
Certificates, Licenses, Registrations:
Must be licensed or eligible for licensure as a Registered Nurse (RN) in the State of Wyoming
Necessary Special Requirements
PHYSICAL WORKING CONDITIONS:
As a condition of employment, the successful applicant employed will be required to execute a release of personnel records to the director/board/commissioners in the county area involved on an as-needed basis. In addition, the successful applicant will be required to complete an Employee Health Assessment, which includes verification of immunity to vaccine-preventable diseases. If found to be non-immune, vaccination will be required unless medically contraindicated. Yearly influenza vaccination is also required, unless medically contraindicated. COVID-19 vaccination or an approved exemption may be required as the position provides services in CMS-regulated facilities. A background check, including the taking of fingerprints, will be completed through the Wyoming Department of Criminal Investigation, the Department of Family Services, and the Federal Bureau of Investigation.

One year of service in the position must be completed. Failure to complete this requirement will require reimbursement of the bonus with the funds deducted from the final pay.
Internal candidates will be required to remain in the position for a minimum of (1) year from the promotion date.
Employees will only be eligible for (1) hiring bonus in a 12-month period
