Atlantic Health System Home Care & Hospice Coordinator in United States

Responsible for working closely with hospitals and other facilities to coordinate continued patient care with hospice home healthcare services after discharge. Assures quality customer service to patients, families, referral sources and co-workers. Serves as a consultant, educator, and customer service specialist to referral sources.

Principal Accountabilities:

  1. Develops initial plan of care for a Hospice admission to Home Hospice, or to a General Inpatient bed.

  2. Coordinates with patients, families, physicians, and discharge planning personnel by obtaining complete clinical information's for the hospice admission.

  3. Contacts the Hospice Manager, as needed to review the appropriateness of General Inpatient Hospice.

  4. Communicates the levels of care for Hospice to the patients and families, and have the patient admitted to the appropriate level of care.

  5. Obtains an initial Hospice order from the referring physician and contacts one of the Hospice Medical Directors for certification of terminal illness for admission to General Inpatient Hospice.

  6. Maintains current knowledge of clinical issues, regulatory and reimbursement standards, insurance contracts and negotiated rate amounts. Acts as a resource to patients, families, physicians, and discharge planners.

  7. Performs other related duties as assigned.

Preferred:

  1. 3-5 years experience in Home Care, Hospice or relevant area.
