Your Specific Responsibilities:

Incumbent in this high-level, independent position will submit grant proposals, implement quality improvement projects, provide employee training, and assist in the completion of annual reports and foundational documents (Community Health Needs Assessment, Community Health Improvement Plan, Strategic Plan, Performance Management / Quality Improvement Plan, Workforce Development Plan, and Emergency Operations Plan) revisions by deadlines set by Central Office.

Research and conduct training courses for employees and/or community members.

Identify grant opportunities, write, and submit grant proposals. Manage grant timelines and deliverables.

Engage employees in formal quality improvement projects.

Attend community meetings and engage in discussions aimed at improving population health.

Ensure paper satisfaction surveys are entered into SurveyMonkey. Create and disseminate weekly and quarterly survey results.

Collect data to update the Performance Management Dashboard.

Track the implementation of the Community Health Improvement Plan, Strategic Plan, Performance Management / Quality Improvement Plan, Minority Health Plan, and Workforce Development Plan.

Serves as secondary Public Health Accreditation Board (PHAB) Liaison. Ensures communication and documentation is submitted to State Office on accreditation updates including participation in statewide workgroups to ensure compliance with PHAB requirements.

Serves as secondary PMQI team liaison for DOH-Brevard in the Central Florida PMQI Consortium. Ensures communication and documentation is submitted to State Office for projects, including, but not limited to QI projects, PHAB, Strategic Plan, Community Health Improvement Plan, Community Health Needs Assessment, Workforce Development Plan, and Emergency Operations Plan in the absence of the primary PMQI liaison.

Other duties as assigned.

Required Knowledge, Skills, and Abilities:

Knowledge of quality management practices, including Six Sigma, and Kaizen. Knowledge of the principles and techniques of effective communication. Knowledge of Microsoft Office software. Ability to collect, evaluate and analyze data. Ability to prepare correspondence and administrative reports. Ability to understand and apply applicable rules, regulations, policies and procedures. Ability to utilize problem solving techniques. Ability to work independently. Ability to plan, organize and coordinate work assignments. Ability to communicate effectively in both written and oral communication. Ability to establish and maintain effective working relationships. Knowledge of basic mathematics. Ability to review, analyze, and evaluate financial data.

Qualifications:

Minimum - Lean Six Sigma Green Belt Certification.
