Returning Citizen Community Program Case Management

The Rescue Mission of Trenton is seeking aReturning Citizen Community Program Case Manager for our Emergency Shelter Unit. Under the direction of the Director of Emergency Services, the RCCP Case manager will provide direct management services to individuals who have a history of incarceration and are experiencing homelessness. We provide a safe, clean, warm refuge for the homeless, the hungry, the transient and the addicted. What it’s like to work at the Rescue Mission of Trenton: Rescue Mission of Trenton was founded in 1915 and is a non-profit organization that helps rescue people from themselves and their situations. Our dedicated staff in the Shelter and our Behavioral Health Unit offer care, support and encouragement to all that reside here. We value dignity, communication and helping each of our clients reach their full potential. Clients/residents are assisted by staff with treatment options, medical help, job placement, financial planning, education, counseling and mental health treatment. Our staff works diligently with the clients to get ID, an income and the things necessary to help them with transitioning to housing.

Qualifications For Position A Bachelor’s degree required and a Master’s in Social Work or related field preferred

Candidate must have a m inimum of three (3) years experience in case management. Preferably prior experience with re-entry population and with homeless individuals.

Should be an active learner with great listening skills, critical thinking, monitoring, reading and written comprehension, and understanding of addictions

Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) preferred

Must be able to operate a computer, be proficient in Microsoft Office (word and excel), Homeless Information Management System (HMIS) and must have a valid driver's license.

Other duties and responsibilities include being able to prov ide direct case management services and support to individuals and clients, such as managing referrals and resolving issue.

Must be able to c ollaborate with key partners, including financial, securing housing, substance abuse treatment plans, and criminal justice agencies, in accordance with Rescue Mission policies, procedures, guidelines ad applicable state regulations

Provide direct counseling and supportive services to assigned clients and be able to p articipate, and encourage client's to participate, in a team approach to treatment including coordinating client services with other Rescue Mission staff and other provider agencies Candidate should be able to assess individuals with a criminal history and their need for substance abuse treatment services, d evelop, implement, review, revise treatment plans under supervision and m onitor client outcomes Documentation skills are necessary to document to all licensure standards and established policies and procedures, must be a quick learner and able to maintain client charts, roster, records, and reports. Should understand, research and analyze community needs to provide program direction and goal recommendations while bein a dvocate on client behalf, policy, and program level Must participate in staff meetings, determine the organizational policies regarding eligibility, program requirements, and program benefits, c ommunicates program trends to supervisor, p articipate in ongoing training and education to maintain or obtain licensures/certifications Annual Salary: $44,800 Schedule: Tuesday and Thursday 12:00pm to 8:00pm; Wednesday, Friday and Saturday 9:00am to 5:00pm

Employees who are not eligible for a booster at the time of an offer of employment provide proof they have received a booster upon eligibility and upload proof of same. Failure to provide proof of primary vaccination and booster will result in recession of a candidate’s offer of employment or disciplinary action up to and including termination.
