Summary:

The Trauma Coordinator is a unique position and serves as a liaison between The Trauma Service and the nursing units as well as consultants and

referring physician. Reports to the Trauma Program Manager and is responsible for all phases of the QI process, special studies, analysis of data, reports and presentation of QI data as well as participates in patient throughput and medical documentation.

Knowledge and Abilities:

  • A Bachelors degree in nursing, business or other healthcare related field is prefer.

  • Must possess current license as a Registered Nurse rom the Sate of Ohio, CPR-trained required.

  • Must possess a minimum of five years experience in critical care nursing (ICU, CCU, Emergency or Trauma).

  • Demons leadership skills and expertise in planning, organizing and materials for reports, etc.

Working Conditions:

  • Works primarily in and office environment as well as normal patient care environment where there is some exposure to communicable diseases.

  • Work requires long periods of time using a computer and regular walking, standing and sitting.

  • Considerable mental/visual effort required in reviewing quality audit filters. organizing materials for reports, etc.

Leadership Responsibility:

  • The Trauma Coordinator is a nurse whose role is to serve as focal point for all quality improvement processes as well as patient care in theTrauma Program.

  • Coordinates trauma registry staff activities.

  • Liaison to referring Hospitals, EMS Agencies, and the community.

Job Duties & Responsivities:

  • Develops, monitors and evaluates the annual trauma performance improvement plan for the Adult and Pediatric Trauma Service.

  • Monitors, evaluates and reports clinical outcomes and system: issues related to trauma quality care. (PL Case Review, Queries, Audi Filters, Of projects)

  • Responsible for data any for all QI issues, trauma queries, PI Case Reviews, Peer Review and loop close.

  • Identifies trends and sentinel events and helps outline remedial actions while maintaining confidentiality.

  • Analyze trauma data, prepare reports, and presents at the appropriate venue.

  • Stimulates practice change, education and protocol development based on the literature, research, quality monitoring and benchmarking data.

  • Assists with the coordination of education program and research.

  • Collaborates with other hospital departments (infection control, BT, and Risk Management) regarding trauma issues and loop closure.

  • Coordinate registry personnel activities to meet report deadlines and assure timeliness and accuracy of data.

  • Continually evaluates the Quality Improvement process looking for efficiencies in process.

  • Responsible for the operation of Trauma MM as well as Trauma Committee and Trauma Grand Rounds.

  • Facilitates the development of safe/effective plans through early identification of patient needs and possible benefits and resources available.

  • Assures appropriate sequencing of care/interventions for optimal results and smooth transition along hospital care and continuum.

  • Demonstrates technical and clinical competence in specialty area and serves as clinical resource person and role model for stall.

  • Coordinates discharge plan related to patient needs and payor requests. Assures patient education is complete and documented.

  • Maintains a current working knowledge of trauma patient care and works with Leadership to maintain quality levels of trauma care.

  • Coordinates Trauma Program Performance Improvement activities and sees that they are incorporated with hospital and medical staff performance and improvement processes.

  • Acts as liaison between patient, physician, family and health care team regarding plan of cars and progress towards goals.

  • Acts as a liaison between referring facilities, EMS Agencies and the community.

  • Coordinates documentation efforts of physicians, nurses and team members; reviews and transmits data to appropriate parties.

  • Coordinates physician consults as needed.

  • Performs patient rounds to evaluate the patient, test and answer any patient, family, or nursing questions.

  • Prepare discharge planning including but not limited to Rx's, CRF's, and follow up care as appropriate.

  • Assist in trauma education as needed.

  • Adheres to the standards and policies of the Corporate Responsibility Program, including the duty to comply with applicable laws end regulations, and reporting to.designated Manager (or employer hotline) any suspected unethical, fraudulent, or unlawful sets or practices.

  • Shift: Days

  • Hours per pay period: 40

  • Weekend, holiday and on-call rotation required per department policy
