Job Description

Medicaid is a state-federal funded program that is administered by the Department of Health and Human Services (HHS) and is the second largest insurance program in the State of Iowa. Iowa Medicaid establishes the state’s policy to oversee performance and compliance of the Medicaid Managed Care program

Under the general supervision of the Iowa Medicaid Long-Term Services and Supports (LTSS) Policy Bureau Chief, this position will serve as a regional resource for community integration activities including high level case review, consultation, and guidance for individuals transitioning from institutional care facilities in geographically assigned areas of Iowa. This position will be responsible for conducting comprehensive case reviews, participating in multi-stakeholder team meetings, and providing technical assistance to address critical needs of individuals who are transitioning into new community-based care settings.

This position will incorporate review of existing documentation including, but not limited to, critical incident reporting, person-centered service plans, needs assessments to identify facility residents who may require heightened attention to ensure a successful transition to community-based service settings. This position will provide both consultative services and ongoing compliance monitoring for the development of individualized, comprehensive person-centered transitional care plans through collaboration with the Interdisciplinary Team (IDT) including facility residents, families, guardians, Managed Care Organizations (MCOs), Medicaid personnel, facility personnel, Mental Health and Disability Service (MHDS) regions, community-based providers and others to provide support and to ensure service planning needs are addressed during the transition process.

This position will require extensive knowledge of Iowa’s Home and Community-Based Services (HCBS) programs and policies. In addition, this position will require knowledge of other statewide services available to individuals with disabilities including Iowa’s LTSS system, Medicaid and MHDS Regions.

This position will be required to become proficient in the use of HHS systems including the Iowa Medicaid Portal Access (IMPA), IoWANS, Medicaid Management Information Systems (MMIS), and other systems and required.

Successful candidates will be able to think critically and analyze problems, as well as apply quality assurance approaches strategically. Candidates with experience in Medicaid LTSS/HCBS, Targeted Case Management, State Plan Habilitation, and clinical program or project management, will be given priority.

546 Intellectual Disability and Developmental Disabilities

6 months’ experience in an intellectual disability and developmental disabilities environment in order to identify measure and outcome indicators for ID/DD services and work with management to improve operational plans for clients [this was written for a position at the Woodward Resource Center in April 2003] - competencies gained from such experience would include the ability to design processes for the review of data, make decisions based on data and on trends and regulations in providing services and support to people with ID and DD; knowledge of types of and trends in services to support people with ID and DD; and knowledge of applicable rules and regulations that govern the operation of services provided to persons with ID and DD.

Minimum Qualification Requirements

Graduation from an accredited four year college or university and the equivalent of four years of full-time experience in a social work capacity in a public or private agency;

OR

a Bachelor's degree in social work from an accredited four year college or university and the equivalent of three years of full-time experience in a social work capacity in a public or private agency;

OR

a Master's degree in social work from an accredited college or university and the equivalent of one year of full-time experience in a social work capacity in a public or private agency;

OR

an equivalent combination of graduate education in the social or behavioral sciences from an accredited college or university and qualifying experience up to a maximum of thirty semester hours for one year of the required experience.

OR

employees withcurrentcontinuous experience having earned their full-time equivalent social work experience with the Iowa Department of Human Services may qualify with one year less of the required experience.
