Mercy Home Health, a member of Trinity Health at home has an immediate opening for an AB Collaborative Program Coordinator.

The AB Collab Program Coordinator must be a licensed PT or OT in their respective state. The Coordinator is responsible for working with agency operational leadership to ensure safe, effective, and cost-efficient utilization of therapy visits that aligns with financial targets. The patient population resides in Assisted Living Communities and the therapy services are provided by our collab partnerships with the Part B on-site therapy provider. The Program Coordinator will organize on-boarding of new partners and obtain necessary information for security clearance, equipment procurement, provide orientation to new therapists, ensure timely completion of all mandatory education modules, and serve as the clinical resource for rehabilitation practice and therapy case management for the Collab partners. The Coordinator will provide supervision and guidance to the therapy service team, staff, and contractors in relation to quality, regulatory and clinically based issues, and will address and resolve operational issues/concerns that arise with daily ops with the AB Collab service line. This candidate must have a current working knowledge of regulatory developments and changes in relation to rehab services in the home care setting, and keep abreast of all current industry developments associated with the rehab professions of Physical, Occupational and Speech Therapy.
