Responsibilities - Case Manager - BH:
Provide linkage, monitoring, planning and advocacy for the most appropriate mental health and community medical and social services.
Coordinate the service delivery system for patients.
Provide residents, faculty and community mental health practitioners with up-to-date available referral resources to provide continuity of care.
Perform other duties as assigned.
Qualifications:
Required: 1 year - Social Work
Or, 1 year - Case Management
Education:
Required: Bachelors - Social Work
Or, Bachelors - Counseling
Or, Bachelors - Human Services
Or, Bachelors - Psychology
