  • 40 hours per week

  • Monday – Friday

  • Remote work 2 days per week AFTER 6 month probationary period

  • Provides professional social work services in a primary care setting

  • Provides social work services to clients/patients, their families, and/or others, all while exercising independent decision-making authority and expertise specific to assigned area of practice. UW Medicine board-certified providers offer a complete spectrum of primary care services five days a week, including family medicine with behavioral health, travel medicine, nutrition services, reproductive health and sports medicine.

PRIMARY JOB RESPONSIBILITIES

  • Works in collaboration with a multidisciplinary team to provide psychosocial support and support utilization guidance to patients and families

  • Provides care coordination for patients with complex social and medical histories

  • Makes timely referrals to hospital and/community agencies, including those required by mandatory reporting laws such and child and vulnerable adult abuse

  • Participates in practicum instruction of social work students and provides work direction to Social Work Assistant(s) and other staff

  • Engages in departmental program development activities, tasks forces, and committees

  • Provide crisis intervention to patients both over the phone and in person

  • Provide psychosocial assessments along with individual and group therapy utilizing evidence-based interventions REQUIREMENTS

  • Masters of Social Work Degree from a program accredited by the Council on Social Work EducationAND

  • One year of full-time social work experience in a health care setting or equivalent.(Experience can include a practicum placement as part of a social work training program and/or employment experience.) WASHINGTON STATE LEGAL REQUIREMENT Within 90 days of hire must hold one of the following credentials issued by the state of Washington:Agency Affiliated Counselor, or

  • Licensed Social Work Associate - Advanced, or

  • Licensed Social Work Associate - Independent Clinical, or

  • Licensed Advance Social Worker, or
