Express Healthcare Staffing Licensed Practical Nurse (LPN) in Seattle, Washington

Job Summary: We are seeking a compassionate and skilled Home Health Nurse to provide personalized 1:1 care to patients in the comfort of their homes. The Home Health Nurse will work closely with individuals of varying ages and medical needs, delivering high-quality medical care, emotional support, and education to both patients and their families. This role requires a strong sense of empathy, excellent clinical skills, and the ability to adapt to different environments and situations.

Responsibilities:

  1. Conduct comprehensive assessments of patients' health status and develop individualized care plans in collaboration with the healthcare team.
  2. Administer medications, treatments, and therapies according to prescribed protocols and physician instructions.
  3. Monitor patients' vital signs, symptoms, and overall condition, and promptly report any changes or concerns to the appropriate healthcare providers.
  4. Provide wound care, catheter care, and other specialized nursing interventions as required.
  5. Offer emotional support, counseling, and education to patients and their families regarding their medical condition, treatment options, and self-care techniques.
  6. Assist patients with activities of daily living, including bathing, grooming, dressing, and mobility, while promoting independence and dignity.
  7. Coordinate with other healthcare professionals, such as physicians, therapists, and social workers, to ensure holistic care and continuity of services.
  8. Document all patient assessments, interventions, and outcomes accurately and timely in the electronic health record system.
  9. Collaborate with community resources and agencies to facilitate patients' access to additional support services and resources as needed.
  10. Maintain strict confidentiality and adhere to ethical standards in all interactions with patients and their families.

Qualifications:

  1. Licensed in WA
  2. Bachelor of Science in Nursing (BSN) preferred.
  3. Minimum of 1 years of clinical experience in nursing, with demonstrated proficiency in home health or related field.
  4. Current Basic Life Support (BLS) certification; additional certifications such as Advanced Cardiac Life Support (ACLS) may be required depending on the patient population served.
  5. Strong assessment, critical thinking, and problem-solving skills.
  6. Excellent communication and interpersonal skills, with the ability to establish rapport and build therapeutic relationships with patients and families.
  7. Ability to work independently and as part of a multidisciplinary team in a dynamic and fast-paced environment.
  8. Proficiency in using electronic health record (EHR) systems and other healthcare technology tools.
  9. Valid driver's license and reliable transportation for home visits.
  10. Flexibility to work evenings, weekends, and holidays as needed to accommodate patients' schedules and ensure continuity of care.

Physical Requirements:

  1. Ability to lift and maneuver patients safely, with assistance as needed.
  2. Prolonged periods of standing, walking, and bending during home visits.
  3. Visual and auditory acuity to assess patients' condition and respond to alarms or emergency situations.
  4. Ability to tolerate exposure to potentially infectious agents and hazardous materials in accordance with standard precautions and safety protocols.
