We are currently hiring a Harm Reduction Case Manager, to work in our Care Coordination Services Agency. As the Harm Reduction Case Manager, you would be responsible for:
Make initial contact with eligible participants
Review assessments and discharge plans provided by Medical Staff, CASAC or other facility staff.
Utilize assessments and discharge planning provided by correctional facility partners to develop a plan of care
Deliver engagement services in accordance with the service/care plan
According to program guidelines, consult with participant’s providers on progress of service/care plan services
Document updates to electronic records, shared documents, and other formats as required by protocols, policies, and/or procedures
Implement harm reduction strategies aimed at helping individuals minimize their immediate health and safety risks.
Provide enrollment into the agencies Syringe Exchange Program
Provide access to Safer Injection Supplies
Provide Opioid Overdose Prevention Training in accordance with agency guidelines
Provide HIV and HCV Screening as necessary
Register for and attend community trainings/workshops to assist with professional development
Submit required documentation consistently by expected deadlines.
Provide community partners with appropriate reporting documents
Perform other duties as assigned by agency Administrative staff.
Harm Reduction Case Manager Minimum Salary: $ 42,000.00
Harm Reduction Case Manager Maximum Salary: $ 42,000.00
Harm Reduction Case Manager Schedule: Monday to Friday, 8:30 am to 4:30 pm
Qualifications:
Bachelor’s Degree in health, education, and/or human services with 1 year of relevant experience
Associate’s Degree in health, education, and/or human services with 2 years of relevant experience
Education requirements may be waived by the agency if candidate demonstrates more significant experience (five or more years) in case management, care coordination or case management along with significant continuing education/training hours during previous employment (minimum of 35 hours a year). Plan for continued formal education is preferred, to work towards degree as stated above.
