Coastal Harbor Health Systems is currently seeking a Tele-Assessment Manager (REMOTE)

Coastal Harbor Health System is comprised of a 114-bed hospital for children and adolescents, providing both short- and long-term inpatient care, and a 50 bed inpatient hospital serving men and women, ages 18 and up. We offer compassionate patient care to those who need it the most, serving not only local patients, but also those located across the country. Job Description:

Participates in On-Call monthly schedule, carrying a functioning cell phone to respond to calls.

Coordinate Assessments and Referrals Services and Mobile Assessments.

Responsible for monthly on-call scheduling, including back-up and shadowing.

Coordinates with Business Development Staff in marketing MAT services

Prepares and completes all monthly reports for Mobile Assessment Team statistics.

Conducts an initial face to face assessment on site, arriving at the site no later than 60 minutes after receipt of the call.

Demonstrates the ability to assess and provide appropriate interventions that are unique to the needs of the patient population (age and program specific) being served.

Develops and implements policies and procedures for mobile assessments, ensuring the overall quality of services provided.

Accurately performs quality improvement audits.

Formulates level of care recommendation and immediate patient/client disposition. Makes arrangements for immediate disposition regarding transfers/discharges. Provides primary contact with patient and family until transfer/discharge occurs.

Serves as a liaison between mobile assessment/crisis intervention and Assessment Referral Services.

Serves as a back-up team for incoming assessment and referral calls.

Reviews safety policies for your department to ensure compliance with regulatory agencies and hospital practice.

Review the number of injuries, types and costs for your department as well as provide effective corrective action which results in reduction of accidents.

Set goals for your department and review every six months for compliance.

Reviews and maintains updated job descriptions, performance evaluations, orientation checklists and competencies.

Report all accidents immediately and provide effective timely corrective action on all accidents.

Will adhere to all safety policies and safe work practice.

Will adhere to all hospital policies and procedures.

Support facility internal and external customer service standards.

Follow Coastal Harbor Health Systems Time and Attendance policy.

Perform other duties as assigned.

Provide input in developing procedures to maintain compliance with all TJC standards.

Assists in promoting the growth of programs through staff engagement and implementation of interventions to determine how to best meet the needs of the patients while fiscally benefiting the company.

Maintains quality improvement data and executes process improvements. Benchmarks best practices and enforces strategic goals.

Measures programming and interventions against expected outcomes and modifies as indicated.

Ensures that outcome measures are completed timely and entered.

Oversite of quality of documentation according policy and TJC standards

    Qualifications

    Job Requirements

    Requirements:

    Master’s level in social work or counseling with at least two years of related experience. Must be fully Licensed - LMSW, LPC, LMFT.
