As the LVN Clinical Coordinator, you will be respobsible for providing clinical support to the agency with the processing of home health orders, lab results and communication. Under the supervision of the Executive Director, the LVN Clinical Coordinator provides clinical coordination and communication of patient care with the field clinicians, physicians, referral sources, and sales team members, maintaining the highest degree of quality care, customer service, and regulatory compliance.

  • Provides clinical coordination support to ensure efficient and timely care delivery within standard operating procedures. Tasks include but not limited to:

  • Working with the Scheduler to assign patient visits

  • Transcribing communication, telephone encounters, faxes, physician orders, and lab results between various electronic medical record systems

  • Following up after-hours patient or clinician communication

  • Communicating information to physicians, clinicians, and patients as needed

  • Updating medications lists

  • Faxing and obtaining orders

  • Upon request of the manager, provides intermittent support to clinical intake department with tasks including but not limit to:

  • Confirming that the patient referral meets home health eligibility requirements

  • Confirming a physician to follow for home health orders

  • Calling patients to confirm acceptance of service

  • Retrieving and documenting orders

  • This position includes travel up to 10-15% to locations

  • On-call, weekend shift: weekend rotation

  • Patient visits as needed upon request by the Executive Director

  • Current Licensed Vocational Nurse in state of employment

  • Current CPR certification

  • A licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order

  • Knowledge and ability to effectively provide coordination of patient care consistent with standards and regulations

  • Must have exceptional communication and customer services skills and be able to effectively problem solve and identify when to consult the Executive Director

  • Knowledge of home health services and routine clinical practices, Medicare Conditions of Participation, home health accreditation requirements, reimbursement patterns for Medicare and other payors
