Kaiser Permanente Patient Coordinator (Bilingual Cantonese) in San Francisco, California

Job Summary:

THIS JOB MUST BE REVIEWED WITH TOTAL COMPENSATION BEFORE POSTING. Coordinates the provision of program services for members in conjunction with a multidisciplinary team to determine if members are eligible for services. Patient Coordinator functions include assisting patients requesting services on their own, patients being referred for consultation from other departments (clinic or hospital) and patients who are in the emergency room requiring urgent evaluation.

Essential Responsibilities:

  • Assists multidisciplinary team by collecting patient demographics, as well as health plan membership and benefit information. May include frequent use of telephonic support to respond to patient, physician and staff inquiries.

  • Assists in coordination of care as directed by a multidisciplinary team or other departmental clinicians.

  • Coordinates appropriate evaluation service with clinicians both inside and outside of all applicable departments.

  • Assures that patients and physicians follow appropriate processes, and complete necessary paperwork as required by law and KP policy.

  • Develops and maintains complete records of patient evaluation; collects non-clinical data regarding patient requests for services (i.e how to access services, housing, etc.), and participates in record management for e- Consult and patients participating in the program.

  • Provide timely response to inquiries and informs patients and referral sources of limitations or scope of services available.

  • May attend clinical team meetings and liaison meetings with department.

  • Coordinates with multidisciplinary staff, including psychiatrists, psychologists, social workers, case managers, patient assistance workers, nurses, Emergency Room staff, hospice and palliative care team, EAP.

  • Works with other managers, administrators and physicians regarding departmental operations related to record management and coordination of care.

  • Maintains roster of program participation and coordinate patient records appointments.

  • Reviews e-Consults , escalates issues as needed, and closes out e-Consult.

  • Coordinates all paperwork for team, including contacting patients and sending forms prior to appointment.

  • Coordinates new patient packets and sends to patients as appropriate.

  • Kaiser Permanente conducts compensation reviews of positions on a routine basis. At any time, Kaiser Permanente reserves the right to reevaluate and change job descriptions, or to change such positions from salaried to hourly pay status. Such changes are generally implemented only after notice is given to affected employees.

Basic Qualifications:

Experience

  • Minimum two (2) years of healthcare experience to include telephone screening of individuals requesting medical care within treatment modalities provided.

Education

  • Bachelors degree OR four (4) years of experience in a directly related field required.

  • High School Diploma or General Education Development (GED) required

License, Certification, Registration

  • N/A

Additional Requirements:

  • Demonstrated strong interpersonal communication skills including problem-solving and decision-making.

  • Demonstrated ability to work well with physicians and other mental health professionals in a multidisciplinary treatment setting.

  • Must be able to work in a Labor/Management Partnership environment.

  • Bilingual (Cantonese) Level I required.
