We invite you to join our Hematology/Oncology team as a Specialty Nurse Navigator.
As a Specialty Nurse Navigator, you will be responsible for:
v Formulating custom care plans from assessment and clinical information demonstrating the application of clinical guidelines including standards of care. v Establishes specific, measurable, attainable, relevant and time specific nursing goals to guide the care planning process and promote self-reliance. v Assists patients in establishing, modifying, or reinforcing a social support network, thereby promoting self-reliance. Identifies and aligns resources that impact high-risk care. v Coordinates services on behalf of the high-risk patients. v Work in collaboration with the Healthcare team to track and review data for research projects as well as resource utilization related to relevant specialty and make recommendations as needed. #LI-MR1
Required Education/Experience/Specialized Skills:
· BSN 3 years recent clinical nursing experience in a variety of clinical settings.
Required Certification/Registration:
· CA RN License
· BLS if working in a clinical setting.
· ONS Chemotherapy and Biotherapy if working in Hematology.
Preferred Education/Experience/Specialized Skills/Certification:
· MSN · OCN if working in Hematology ONN-CG if working in Oncology area. · Two years experience managing care of relevant specialized patient population.
