Hourly Pay Range (Minimum - Midpoint - Maximum):

$80.979 - $104.489 - $127.999

The stated pay scale reflects the range that Sharp reasonably expects to pay for this position.  The actual pay rate and pay grade for this position will be dependent on a variety of factors, including an applicant’s years of experience, unique skills and abilities, education, alignment with similar internal candidates, marketplace factors, other requirements for the position, and employer business practices.

Required Qualifications

  • Other : Education pursuant to licensure requirement.

  • California Pharmacist License - CA State Board of Pharmacy -REQUIRED

  • AHA Basic Life Support for Healthcare Professional (AHA BLS Healthcare) - American Heart Association -REQUIRED

Preferred Qualifications

  • 1 Year experience in a hospital pharmacy environment.

Other Qualification Requirements

  • AHA BLS is required within 90 days of hire.

Essential Functions

  • Achieves patient and pharmacoeconomic outcomesCommunicates with nurses and physicians, in a timely manner, information regarding therapeutic issues or problems.Demonstrates knowledge of and has active participation in the outpatient pharmacy quality assurance program: medication error monitoring and reporting.Qualitative work demonstrates thoughtful and thorough identification, evaluation and analysis of triggers / medication use issues that might contribute to adverse drug events and medication errors.Quantitative work reflects the desire to contribute to improvements in safe medication practice and includes the completion and submission of quality assurance data collection forms that contribute to the outpatient pharmacy quality assurance program.

  • Medication dispensingMedications are prepared and dispensed accurately and in a timely manner in accordance with recognized standards of practice, legal and regulatory requirements, and pharmacy policies and procedures.Enters data correctly into computerized patient medication profileMedications are prepared and dispensed accurately and in a timely manner in accordance with recognized standards of practice, legal and regulatory requirements, and outpatient pharmacy policies and procedures.Selects, assembles and compounds all ingredients in the required amounts according to master formulas and special requests, using an extensive knowledge of ingredient properties and characteristics, computational accuracy, measuring systems and devices, and sterile technique.Understands and maintains knowledge of contracted third party formularies and plan limitations. For medications not covered on third-party Drug Formularies, every attempt is made to have the medication changed to a Formulary Drug item.Assures controlled substances are dispensed with accuracy. Maintains integrity of all record keeping. Assists with narcotic inventories as required.Compliance with regulations on narcotic prescriptions is closely followedPrioritizes stat orders and assures timely completion of prescriptions.Maintains an organized, clean and safe working environment.Assumes daily responsibility for maintaining financial performance within the department by providing clinical input on cost-effective therapy and assuring daily flexing of staff hours.Demonstrates knowledge and competency of IV Admixture/Aseptic Technique including the standards of USP <797>.Demonstrates knowledge and competency of chemotherapy preparation.Demonstrates proficiency with and utilization of automation and advances in technology (Carecast, Clinicomp, Mysis, Questlabs, etc

  • Quality assuranceReviews patient medication profiles for appropriateness of drug therapy, therapeutic duplications, allergies, weight, height, minimum and maximum dosing guidelines, age specific requirements, renal and hepatic function drug interactions and other pertinent factorsClarifies / verifies / communicates therapeutic issues or problems with prescribing doctor / clinic in a timely manner and documents interventions via a Pharmaceutical Care Plan and progress note.Verifies telephone refill orders with original prescription and / or prescribing physician and performs follow-up with patient.Responds to drug information requests promptly and professionally.Counsels patients and customers for proper dose, current medication(s), possible interactions, side effects and / or length of new medication and refills consistent with professional standards and OBRA-90 regulations.
