Hourly Pay Range (Minimum - Midpoint - Maximum):

$68.302 - $88.132 - $107.961

The stated pay scale reflects the range that Sharp reasonably expects to pay for this position.  The actual pay rate and pay grade for this position will be dependent on a variety of factors, including an applicant’s years of experience, unique skills and abilities, education, alignment with similar internal candidates, marketplace factors, other requirements for the position, and employer business practices.

What You Will Do

This position is accountable for the creation, delivery and coordination to support the development and implementation of programs and services consistent with the mission of the Integrated Care Management Division. Responsible for supporting the strategic plan, business development and growth, and quality improvement plans for the education program for the Integrated Care Management (ICM) service line. Collaborates regularly with other hospital Educators internally, externally and with Sharp Medical Groups. Is a key member of the System ICM leadership team responsible for strategic planning and implementation of the program enhancements. Evaluates and coordinates clinical needs assessments for the ICM specialty area of practice and creates programs to address identified gaps. Coordinates and supports ICM management in the integration of financial, clinical, and quality issues for the ICM specialty service line to achieve high quality service in an efficient and economic manner. Develops collaborative relationships with key members internally and externally to ensure a continuum focus and enhance the image of ICM service line and Sharp HealthCare. Provides leadership in forecasting future trends or needs for service lines. Key contributor to maintain the integrity of the overall ICM Program to meet the Joint Commission, County of San Diego, and State of California regulatory requirements.

Required Qualifications

  • Master's Degree in Social Work.

  • 5 Years clinical experience.

  • 2 Years clinical experience in an appropriate healthcare setting.

  • 3 Years leadership experience and experience with education creation, development and delivery.

  • California BBS Licensed Clinical Social Worker (LCSW) - CA Board of Behavioral Sciences -REQUIRED

Essential Functions

  • Business and program developmentIdentifies ideas and insights into clinical improvements to generate new business or improved services for service lines. Participates/facilitates in the development of Sharp HealthCare ICM service line continuum, ensuring the incorporation of quality and educational program needs to optimize the ICM financial performance. Maintains ongoing analysis of ICM service line performance and monitors trends and opportunities. Develops, executes and evaluates evidenced-based education programs to meet changing needs of the organization at both the local and system level. Incorporates knowledge of external factors, national social work and case management trends and healthcare policies, into ICM educational programs. Collaborates with ICM Directors and Managers of system and local departments to provide strategic planning and implementation for new services. Leads focused audits to assist in identifying areas of opportunity for growth and staff education. Other duties as assigned.

  • LeadershipCollaborates on a routine basis with the other system ICM Educator along with the ICM leaders at the regional and metro facilities.Facilitates information sharing and project coordination where appropriate between facilities. Develops collaborative relationships with management, clinical staff, and other healthcare team members to facilitate patient, case management, and organizational outcomes. Establishes and maintains a communication system with individuals/agencies providing or supporting the ICM service line and program development. Uses communication strategies with individuals and groups that result in achievement of intended outcomes. Facilitates the integration of appropriate personnel into high performing teams. Advances integrated care management practice through participation in professional organizations, publications, and presentations. Participates in professional social work organizations and partners with community organizations and schools. Acts as a resource to staff social workers to improve the quality of patient care as it pertains to the discharge planning process. Promotes an integrated case management, clinical social work model that emphasizes professional role accountability, critical thinking, and collegiality with physicians and other members of the interdisciplinary care team. Expands the practice of case management through ongoing generation and acquisition of scientific knowledge. Provides leadership as role model and/or consultant to develop/retain professional social work staff. Completes annual competencies for staff to identify areas of staff growth and development.

  • Staff educationUses advanced theoretical knowledge and clinical expertise to act as a staff resource and under the direction of the Director/Supervisor/Lead to develop and create department specific education. Consults with staff or members of the interdisciplinary team on clinical issues to achieve optimal patient outcomes. Evaluates staff clinical competencies and works with the Integrated Care Management Leadership to develop staff education.

  • Training and educationCreates social work specific onboarding tools and annual competencies. Effectively provides ongoing constructive feedback, teaching, training, and support to all ICM service line staff, and ICM management. Provides education classes to ICM staff on identified needs or new program initiatives or requirements. Seeks input from staff in the development of action plan to improve their performance. Collaborates with other departments to develop an education plan in order to increase staff compliance and engagement on all applicable metrics.Provides direction, oversight and collaboration with nursing and interdisciplinary colleagues (educators, senior specialists, shared decision-making councils, etc.) in assessing the educational needs and designing educational programs that: a) Develop competency-based orientation programs and ongoing clinical nursing competency, b) Develop efficient and innovative processes to advance the practice of case management and discharge planning, and c) Foster the integration of the professional role of case manager, and other non-clinical roles within the department and promote acquisition of complex technical skills. Evaluates outcomes of staff education for effectiveness of knowledge transfer and cost-benefit.
