State of Massachusetts Human Services Coordinator I North Shore Site Office in Salem, Massachusetts

The Department of Mental Health (DMH) is seeking qualified professionals for the position of Human Services Coordinator to promote services that are clinically appropriate, least restrictive and delivered close to home. The services are designed to be strength-based, recovery-oriented and culturally/linguistically competent. The selected candidate advocates for client’s rights with all community providers, agencies, families, court systems, employers and significant others to ensure that client’s rights are recognized and respected. Please note: This position is full time 37.5 hours per week, 9:00am-5:00pm This position travels to meet with clients and family members outside the office in various settings such as homes, hospitals, programs, and other accessible community settings. Candidates will need access to a reliable motor vehicle for transportation. /_Duties and Responsibilities (these are a general summary and not all inclusive):_/ * Develops comprehensive assessment of client needs by interviewing client, obtaining records and information, reviewing referral information, and interviewing clients to evaluate functional level and service needs. * Provides supportive counseling, outreach, and education to clients and family members to assist them in identifying, accepting, and obtaining the necessary services. Is instrumental in maintaining and enhancing existing skills and resources, and/or changing non-adaptive behaviors. * Maintains an assigned caseload by meeting with clients on a regular basis, ensuring that financial entitlements and health insurance are obtained, acting as an advocate to clients and their families, and ensuring client access to services. * Develops, coordinates and monitors client service plans with clients and service providers; chairs and convenes meetings to review and modify the service and risk management plans as needed; recommends and implements the referrals and transfer of clients to other programs and services. * Utilizes and integrates the Management Health Information System (MHIS), effectively, into the job duties described above in performing functions related to the position of Human Service Coordinator. (Eg: Records and maintains accurate data in client electronic record; to include demographic data, appropriate recommendations for services, complete Medicaid billing and turnaround document; documented encounters and progress notes within the required guidelines established by DMH and MHIS.) * Facilitates and/or participates in client review meetings such as hospital discharge planning, program client reviews, Treatment Team meetings, and At Risk meetings, in order to monitor client service plans and make recommendations for other services that will meet client needs. * Assess potential crisis and risk level of client and collaborates with client, crisis team, and service providers to facilitate appropriate planning. Assists clients in acute crisis by facilitating timely connection with appropriate crisis intervention service. * Refers clients to programs and services and facilitates their adjustment by completing required forms, advocating for clients with programs and services, education clients about procedures, and by transporting client when necessary to the required services to ensure the implementation of service plans * Maintains client records by requesting all relevant client reports, documenting progress notes, termination and interval summaries, risk management plans, meeting minutes and regularly updating changes in client records to ensure completeness and accuracy. * Serves as a representative of the Department in order to foster public relations by maintaining good working relationships with service providers, community agencies, hospitals, and individuals, for the purpose of ensuring a good quality of client care and resource allocation. * Identifies clients at risk, and problems or gaps in service delivery system that impact on client needs and access to services and provides feedback to supervisory staff for planning purposes. * Meets with assigned clients on a regular basis according to individual service needs. * Ensures assigned clients obtain and maintain financial entitlements and health insurance in which they are eligible and act as an advocate for clients and their families to ensure access to services. * Participates in regular supervision meetings and participates in trainings and workshops that will enhance case skills. * Complies with HIPAA regulations and confidentiality standards regarding the disclosure of protected client information. * Is accountable for time management and observes proper office procedures. * Performs other duties as assigned. /_Required Qualifications:_/ · Demonstrated knowledge of psychosocial problems of those with mental/emotional disorders and the types and symptoms. · Knowledge of the principles, practices and techniques of counseling. · Familiarity with the types of programs and services available to individuals living with mental illness. · General understanding of interviewing techniques. · Knowledge of methods of general report writing with the ability to assess case management process and results. /_Preferred Qualifications:_/ * Strong and effective interpersonal skills, including the ability to listen well, relate to others and demonstrate sensitivity and respect for individuals. * Demonstrated commitment to serving a diverse population with cultural competence and sensitivity, as well as the ability to work with a wide range of constituencies with diplomacy and tact. * Proficiency in Microsoft Office applications including Outlook, Word, and Excel. * Given the population served, bilingual or multilingual fluency. /_DMH Mission Statement:_/ The Department of Mental Health assures and provides access to services and supports to meet the mental health needs of individuals of all ages, enabling them to live, work and participate in their communities.
