Primary Function of the Position: Hmong American Partnership is seeking a Wellness Senior Coordinator to support the success of its Community Impact programs centering around COVID-19 recovery and housing stability for the Southeast Asian and immigrant/refugee populations. The Wellness Senior Coordinator is expected to understand the workplan objectives of these key areas, understand the interdependencies of each program to the larger mission of HAP, and effectively support the unique outcomes of each program. This position will be responsible for the oversight of meeting the COVID-19 program objectives, researching COVID-19 impacts and policies, budget, evaluations and reports requirements, including researching, developing and facilitating program content. The Wellness Senior Coordinator will be responsible for providing support in meeting the Housing Stability program objectives under the direction of the Housing Stability Case Manager and the Director of Community Impact. The Wellness Senior Coordinator will work closely with HAP staff and external partners to strengthen relationships through community building and outreach. Position Responsibilities: Effectively track and manage program and performance goals and outcomes to ensure success Research and analyze COVID-19 impact and recovery data and policies to inform program services Lead evaluation and continuous program improvement development and implementation, including compliance Coordinate, develop and implement program activities, including facilitation, making referrals and providing resources Maintain the reporting and accountability necessary to ensure program compliance Research, develop and deliver program curricula to participants in group and one-on-one settings Learn and develop appropriate strategies to support program staff of both the COVID-19 and Housing Stability programs. Support includes and is not limited to becoming familiar with program objectives, regularly checking in with program staff to brainstorm programmatic issues and move activities towards outcomes and knowing when to obtain support and feedback from program leaders Ensure compliance with program data integrity and evaluations practices Draft program narratives for reports, proposals and marketing Build and maintain relationships and partnerships with key stakeholders such as community agencies, community members, property managers, and more Support and guide program staff in coordinating services and activities in accordance with workplans and the mission of HAP, including outreach Support and guide program staff in producing accurate and timely reporting of program status Participate in strategic work planning sessions with program leaders to ensure impact area goals are achieved Lead cross collaborations between programs and impact areas in order to achieve HAP s mission and values. Onboard and train staff, interns and volunteers as needed Coordinate and facilitate program services in group and one-on-one settings, including recruiting Assist the Housing Stability Case Manager in successfully meeting program outcomes Manage confidential information Case management Other duties as assigned, and as needed, to further the goals of the program and in alignment with HAP s mission Essential Functions: The following position requirements are represent tive of those that will be encountered while performing the responsibilities of this position.
