Summary This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. Responsibilities MFT is expected to spend half of their time providing evidence-based marriage, family, individual, and group therapy to Domiciliary Veterans and their families at the St. Cloud VAMC MH RRTP and the other half of the time providing the following: Screening/Assessment - The MFT will interview patients and develop a psychosocial assessment of the Veteran. The MFT may gather and evaluate information from sources other than the patient, with the patient's consent, to validate patient reports and provide a more comprehensive assessment. The goal of the assessment is to highlight the Veteran's strengths, limitations, and internal/external supports and service needs in order to optimize the Veteran's functional status. Treatment Planning/Goal Setting - It is the responsibility of the MFT, working within the interdisciplinary treatment team, to develop a treatment plan based on assessment data and the input from the Veteran. The treatment plan will include measurable goals, objectives and treatment methods based upon the patient assessment. The treatment plan will be used to direct a course of treatment. The treatment plan will be completed within the time frame indicated by MH RRTP policy. The plan will be reviewed within 21 days of admission or more frequently if there is a change in the patient's condition or needs. Providing Information and referral - Throughout the course of treatment MFT will maintain expertise on VA and/or community resources. Resource development - The MFT will remain apprised of changing and emerging Veteran needs. The MFT will identify service gaps and collaborate with others within the VAHCS and/or community to encourage development or modification of services necessary to meeting the needs of Veterans. Advocacy - The MFT will recognize potential barriers Veterans may face when attempting to access services and will act as an advocate when it serves the best interest of the Veteran and/or family. When appropriate and feasible, the MFT will educate and encourage the Veteran/family to advocate on own behalf, thus fostering a sense of independence and empowerment. Crisis Intervention - The MFT will make rapid assessments and develop crisis management plans in MH RRTP. This includes the development and facilitation of written crisis plans, suicide risk assessments, collaborative safety action plans, and assessment of homicidal ideation. In the event of a medical or psychiatric emergency, staff will follow the protocol to facilitate safe and supportive placement for a Veteran on the acute psychiatric unit consistent with VAHCS, Mental Health Service Line, and MH RRTP policy. Education/Health promotion and prevention - As appropriate, the MFT will provide health care education and/or referral to qualified health care educators to Veterans and their families. The MH RRTP MFT will provide education to Veterans related to Advance Directives/Living Will's and will facilitate completion for Veterans enrolled in MH RRTP. The MFT functions as a team resource in the completion of Advance Directives/Living Will's for interdisciplinary team to which they are assigned. Coordination of non-institutional and institutional services - The MFT will review the progress notes from the other providers to accurately determine the strengths and limitations of each Veteran being referred for mental health services. The MFT will educate the Veteran, their families and the team of all the options available to them and will collaborate with the Veteran and family on the preferred option. Supportive counseling - MFT will provide the Veterans and their caregivers with ongoing supportive counseling. The purpose of such counseling is to assist the Veteran/caregiver in effectively coping with issues of daily living, including dealing with the psychosocial impact of dealing with issues of physical/mental illness and/or addiction. Administrative responsibilities - The MFT will enter all Veteran/family contacts in the electronic record using appropriate formats and templates. It is the responsibility of the MFT to ensure that urgent medical documentation is available for view by other medical staff, therefore, it is required that the MFT ensure timely cosignature (if needed) by directly contacting the clinical mentor/supervisor or the immediate supervisor to sign off on the medical documentation. Information will be entered in a complete, confidential, and professional manner to insure information on the patient is shared with other VA staff. Candidates must meet one of the following: Hold a master's degree or doctoral degree in marriage and family therapy from a program approved by COAMFTE, OR; Hold a master's degree or doctoral degree in marriage and family therapy from a MFT program from a regionally accredited institution, OR; Hold a master's degree or doctoral degree in a comparable mental health degree (Licensed Professional Mental Health Counselor, Social Work, Psychiatric Nursing, Psychology, and Psychiatry) that meets the current VA qualification standard of that profession. Licensure. Persons hired or reassigned to MFT positions in the GS-0183 series in VHA must hold a full, current, and unrestricted MFT license to independently practice marriage and family therapy in a State. Exception: The Secretary, or his/her designee, may waive the licensure requirement for persons who are otherwise qualified, pending completion of State prerequisites for licensure examinations for a period not to exceed three years from the date of employment on the condition that MFTs appointed on this basis provide care only under the supervision of a fully licensed MFT with an AAMFT approved license. Non-licensed MFTs who otherwise meet the eligibility requirements may be given an appointment as an MFT under the authority of 38 U.S.C. § 7401(3). This exception only applies at the entry grade level (GS-9). For grades at or above the full performance level, the candidate must be licensed. English Language Proficiency. MFTs must be proficient in spoken and written English in accordance with VA Handbook 5005, Part II, Chapter 3, Section A, paragraph 3j. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: Marriage and Family Therapist, GS-9 (Entry Level) Experience, Education, and Licensure. The entry level grade is used for non-licensed MFTs or for MFTs (master's or doctoral level) who are graduates not yet licensed at the independent practice (Journey) level. Unlicensed MFTs at the GS-9 grade level have completed the required education listed above, and are working toward completion of prerequisites for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs: Knowledge of human development throughout the lifespan, interventions based on research and family systems theory and therapy, formal diagnostic criteria, risk assessment, evidence-based practice, and assessment tools. Ability to assess, with supervision, the psychosocial functioning and needs of Veterans and their family members. Knowledge to formulate, implement, and re-evaluate a treatment plan through continuous assessment identifying the Veteran's challenges, strengths, readiness to change, external influences, and current events surrounding the origins and maintenance of the presenting issue, and interactional patterns with the client system. Ability to provide counseling and/or psychotherapy services, under supervision, to individuals, groups, couples, and families in a culturally competent manner that facilitates change through restructuring and reorganizing of the client system. Basic knowledge and understanding of existing relevant statutes, case law, ethical codes, and regulations affecting professional practice of marriage and family therapy. Ability to draft documents, manage data, maintain accurate, timely, and thorough clinical documentation, and track quality improvements. Marriage and Family Therapist, GS-11 (Full Performance Level) Experience, Education and Licensure. In addition to the basic requirements, the GS-11 full performance level requires completion of a minimum of one year of post-master's degree experience in the field of marriage and family therapy work (VA or non-VA experience) and licensure in a State at the independent practice level. Demonstrated Knowledge, Skills, and abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs: Skill to independently assess the psychosocial functioning and needs of Veterans and their family members. Ability to provide counseling and/or psychotherapy services to individuals, groups, couples and families in a culturally competent manner that facilitates change through restructuring and reorganizing the client system. Ability to establish and maintain effective working relationships with Veterans and their families, colleagues, and other professionals in collaboration throughout treatment regarding clinical, ethical and legal issues and concerns. Knowledge and understanding of existing relevant statutes, case law, ethical codes, regulations and VA policies affecting the practice of marriage and family therapy. This includes the ability to assist Veterans and their families in making informed decisions relevant to treatment to include limits of confidentiality. Ability to provide orientation, training and consultation to new MFTs including clinical oversight of MFT graduate students, and/or provide clinical supervision to pre-licensure MFTs. Skill in the use of computer software applications for drafting documents, data management, maintaining accurate, timely and thorough clinical documentation, and tracking quality improvements.
