Henry Ford Health System Chaplain - Hospice Home Care - 40 Hours - Macomb in Saint Clair Shores, Michigan

General Summary:

  • Assesses and identifies the spiritual and emotional needs of hospice patients and families as well as staff and volunteers in accordance with the hospice concept of care.

  • Coordinates with the Interdisciplinary Team to provid patient/family care.

  • Responsible for the implementation and management of the hospice bereavement program.

  • Maintains continuity of care by ensuring patient needs is met.

    EDUCATION REQUIRED:

  • College graduate with a background in pastoral studies or equivalent studies.

    EXPERIENCE REQUIRED:

  • Past experience in dealing with terminal illness and bereavement issues is helpful.

  • On-the-job Training: A 4-8 week orientation period is recommended to familiarize the employee with policies and procedures as they relate to the hospice program.
