HCR Home Care RN, BSN - LHCSA Regional Director - Fulltime - Rochester NY in Rochester, New York

Role and Responsibilities

The LHCSA Regional Director /Director of Patient Services (DPS) supervises the provision of nursing services provided by the Licensed Agency in a selected region. This pers on is available at all times during operational hours and participates in activities with respect to professional services. Collabor ates between Departments to facilitat e and assu re the delivery of high - quality client care.

Essential Functions

  • Supervise the day-to-day clinical operations (including ensuring workflow tasks are completed even during staff absences) and all required cl ient care is completed, including but not limited to:

  • Review and follow-up on complaint forms and all QI reports .

  • Onsite supervision of RN case managers .

  • Oversee the instruction of nursing and other staff in providing quality service and maintaining standards, including in-home visits to direct and evaluate client care .

  • Proactively address operational and clinical practices to prevent client harm and/or organizational deficiencies or inefficiencies .

  • Assure that the client’s plan of care is executed as written and reassessed by the appropriate health care professional , w hen there is a significant health status change in the client’s condition, at the physician’s request and after hospital discharge .

  • Recommend policies, procedures and/or protocols based upon industry regulations and evidence-based clinical practice standards .

  • Monitor Scheduling personnel’s effectiveness to en sure visits are assigned based on nursing competencies and workloads are equitable .

  • Assure that staff perform according to Agency standards, legal/regulatory standards and the Nurse Practice Act .

  • Conduct case conferences (informal, supervisory or interdisciplinary) to discuss client issues, plan of care , and discharge planning including documentation of such conferences via the Case Conference Coordination Note in the medical record .

  • In collaboration with the HR Department and LHCSA Executive Director, participate in recruitment of qualified personnel and implementation of effective strategies to promote staff retention at acceptable parameters established by the Agency .

  • Assist in determining appropriate staffing levels and maintain adequate staffing to meet client requirement s .

  • Approve weekly schedules and ensure productivity standards are met .

  • Review and approve bi-weekly payroll transmittal .

  • Recommend and collaborate on in-service education programs to promote staff development .

  • Maintain nursing knowledge base , to be current with accepted community standards and best practice s.

  • Participate in community awareness programs , as necessary .

  • Understand and effectively interpret operational budgets and respond to negative variances .

  • Create strategies to meet corporate goals including Team objectives that are attainable. Monitor and evaluate effectiveness using measurable criteria .

  • Provision of monthly staff meetings to meet agency communication plan .

  • Adhere to and participate in Agency’s mandatory trainings .

  • Read and adhere to all Agency policies and procedures and follow the Employee Handbook g uidelines .

  • Maintain understanding and effective demonstration of nursing leadership competencies.

  • Display and maintain e motional intelligence .

  • Log onto H ealth C ommerce site weekly, for updates.

  • Maintain up-t o -date knowledge of federal/state regulations and is aware of contracts and terms.

  • Other duties as assigned .

    Education Requirements

  • Bachelor of Science Degree in Nursing , or equivalent home care experience.

    Qualifications and Requirements

  • Current, valid Registered Professional Nurse license issued by NYS Department of Education.

  • Minimum of two years ’ experience in home care .

  • Two years’ experience in nursing within the last 36 - months .

  • Medicare home health experience preferred .

  • Understands payors of home health services.

  • Organized, detail-oriented , and possesses effective communication skills both orally and in writing .

  • Understands the issues related to the delivery of home health care and is able to problem-solve effectively .

  • Capable of prioritizing and handling multiple tasks of critical importance simultaneously .

  • Functions well in an atmosphere of stress and interruption .

  • Projects the Agency’s mission and translates organizational objectives into strategies for action using the agency’s core values .

  • Effective management skills .

    Work Environment

    The LHCSA Regional Director /DPS is primarily in an office setting and may be exposed to outdoor conditions.

    The working conditions are classified as sedentary work:

  • Sedentary work - Exerting up to 10 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Sedentary work involves sitting most of the time.
