SUMMARY
As a Long Term Care Minimum Data Set Nurse (MDS), your duties will include the ability to assess, monitor and document patient care at our facility.
STATUS:Per Diem
LOCATION:Unity Living Center - 89 Genesee Street
DEPARTMENT:Long Term Care
SCHEDULE:Days/Evenings; as scheduled
ATTRIBUTES
Associate’s Degree in Nursing is required; Bachelor’s Degree in Nursing is preferred
Experience in Long Term Care is strongly preferred
Knowledgeable about Medicare and Medicaid payment guidelines, PPS Guidelines, Care Area Assessments (CAAs) and care planning
LICENSES/CERTIFICATIONS
BLS - Basic Life Support - American Heart Association (AHA)
RN - Registered Nurse - New York State Education Department (NYSED)
RESPONSIBILITIES
Completes the nursing portion of MDS within the time frame specified.
Assesses clinical documentation adequacy to validate the MDS.
Make Medicare coverage determinations for new admissions and continuation of coverage decisions for existing patients in coordination with the Compliance and Reimbursement Specialist.
Completes QI reports as requested
Completes Care Area Assessments that lead to Care Plan development
Participates in the interdisciplinary team meetings such as Care Conference, Spotlight Meetings, Quality
Educate clinical staff about the MDS process.
Fill in for the Compliance and Reimbursement Specialist when needed.
Communicates with insurance companies when requested
Completes the MDS Submission process as needed
Is the RN Assessment Coordinator for the MDS
Provides Assessment for MDS’s completed by LPN’s
Knowledgeable in the use of windows based software applications, good communication skills. Ability to work as a team member and to be a resource for clinical staff.
Completes other duties as requested.

Clinical Ladder
Nurses who advance on our clinical ladder receive a promotion at the bedside through growth in their skillset, a new title and monetary recognition up to $3.00/hour
Rapid Pay Increases
Receive rapid pay increases every six months during the first two years of employment, and annual increases thereafter.
Unlimited Certifications
Nurses earn up to an additional $1.00/hour for each certification earned plus a $1,000 bonus for the first certification earned, and $500 for the second certification.
