FULL-TIME [Nonexempt] : 40 hours
Coordinated Care Services Inc (CCSI) (https://positions.paylocity.com/PositionManagement/wizard/2033539) seeks a talentedLead Outreach Workerto contribute their expertise in providing essential business services to our provider organizations and customers. TheLead Outreach Workeris assigned to provide direct leadership to the outreach team, work on behalf of CCSI's customers, schedule and coordinate community outreach, conduct mediations, and lead our "Boy 2 Men" program for young men (ages 12-19), and build community relationship with schools, r-centers, and other community-based youth resources.
CCSI is partnering with Rise Up Rochester to hire aLead Outreach Worker.This position is employed by CCSI and working on behalf of Rise Up Rochester.
In this role, you will be
Mentoring and coaching outreach workers to develop the ability to build trust and relationships with at-risk youth, young adults, and their families.
Leading coordination with team members to manage gaps in scheduled coverage to ensure community outreach is consistently covered.
Conducting neighborhood canvassing, especially in high-risk areas and after shooting.
Tabling at community events
Collaborating with other organizations doing community outreach.
Maintaining and completing documentation and reporting.
Conducting daily outreach debriefing meetings.
Working with the Program Director to conduct team-building workshops.
Assisting the Violence Prevention Caseworker and Program Director in creating safety plans for participants in the wraparound program.
Assisting the Violence Prevention Caseworker and Program Director in conducting Hotel Check-ins for participants in the wraparound program.
Conducting mediations with the community and partnering with community organizations.
Providing other duties as assigned.
Requirements
Highschool diploma/GED and minimum of 5 years of work experience, or equivalent education and work experience (see CCSI's equivalency chart (https://www.ccsi.org/CCSI/media/pdfs/Equivalency-chart.pdf) )
Training in areas such as solution-focused trauma and informed care, Restorative practice, and conflict resolution.
Additional Requirements
- Satisfactory completion of all required background screenings.
