Trillium Health NY Clinical Pharmacist in Rochester, New York
Purpose: The Clinical Pharmacist will collaborate with primary care providers, clinic team members, and pharmacy team members to provide a cohesive care team to benefit the health of patients. The Clinical Pharmacist participates in day-to-day clinical pharmacy operations through collaborative practice agreements three days of the week and participates in the day-to-day operation of pharmacy activities two days of the week. For clinical operations, activities may include, but are not limited to: patient counseling, consultations with provider and nursing teams, treatment adherence reporting and drug monitoring, administration of long acting injectables, and assisting the prescribers in the appropriate ordering of pharmaceuticals and supplies. For pharmacy operations, activities may include, but are not limited to: prescription processing, accurate patient 3rd party billing, dispensing, patient counseling, clinical drug monitoring, customer service, and oversight of inventory.

Provision of Direct Patient Care/Chronic Disease Management
Provide and oversee chronic disease state management utilizing collaborative drug therapy management (CDTM) for clinic patients (minimum of 1 patient every hour, total of 8-10 patients a day while on clinic days).
Upon referral, assess patient, adjust medications, screen for drug interactions and adverse effects, provide disease/medication education, and explore opportunities to simplify &/or decrease the cost of the medication regimen.
Clinical pharmacist will utilize CDTM to prescribe medication adjustment, order labs and work collaboratively with patient’s primary care provider.
Provide support to patients and providers and promote self-management for chronic disease states and/or pharmacologic management, with a focus on patients with commercial or Medicaid insurance, including but not limited to:
Diabetes
COPD, Asthma
Cardiovascular disease (hypertension, heart failure, hyperlipidemia)
Anticoagulation
Medication reconciliation (regularly scheduled and/or as ordered by provider)
Recommend and administer appropriate vaccines
Implementation, coordination, and administration of long acting injectables for HIV, pre-exposure prophylaxis (PrEP), substance use disorder, etc. Documentation in specialty pharmacy program as needed.
- Collaborative Care Team
Collaborate with pharmacy team and provider team to ensure access to medications, including completing data entry of prescriptions when feasible on clinic days.
Work to improve the stewardship of the limited healthcare resources including assisting third party billing specialists with drug prior authorizations as needed.
Collaborating on innovative projects and policies/procedures to improve the quality and cost-effectiveness of pharmacy-clinic operations and prescribing within the organization. Including being available during clinic days to answer any provider questions about appropriate prescribing/cost-effective prescribing.
During clinic days (Monday, Wednesday, and Friday), be liaison between clinic and pharmacy teams to ensure seamless patient care from check in to medication dispensing with a focus on completing required clinic responsibilities.
During pharmacy days (Tuesday and Thursday), be liaison between clinic and pharmacy teams with a focus on completing required non-dispensing functions and direct patient services within the pharmacy.
Provision of Pharmacy Services
Non-dispensing functions
Perform all duties in accordance with the present laws, rules and regulations
Maintain all required documentation relating to the purchase and dispensing of all pharmaceuticals, including controlled substance requirements
Ensure proper packaging and storage requirements for all medications/supplies
Keep and help maintain all record as required by law and regulation that pertain to the purchase and dispensing of all prescription drug products, including controlled substances
Help to maintain pharmacy supply inventory at designated inventory levels, and remove from stock all outdated or otherwise non sellable products
Ensure security policies and procedures are followed at all times to guard against unauthorized entry into the pharmacy’s dispensing and compounding area
Assist in pharmacy marketing
Participate in clinical pharmacy initiatives in clinic operation
Participate in pharmacy department’s continuous quality improvement (CQI) Initiatives
Be a drug information resource for the community
Direct Patient Services
Fill and dispense prescriptions to pharmacy customers in accordance with all Federal, State and local laws and statutes, all accrediting body requirements, and all internal pharmacy policies and procedures
Triage prescription requests to meet any time constraints and deadlines
Comply with all OBRA regulations for patient counseling including but not limited to administration instructions, side effects, proper storage and potential drug-drug and drug-nutrient interactions
Prevent unauthorized fills/refills of prescriptions and unauthorized sale of legend drugs and devices
Ensure for the proper storage, sale, dispensing or compounding of drugs, poisons and restricted devices (e.g. hypodermic needles and syringes)
Input and/or verify all patient demographics, including allergy information, OTC and herbal usage, and all insurance information required for reimbursement
Ensure accurate prescription claim processing /adjudication before dispensing and utilize Third Party Specialists support for any rejected claims or prior authorizations
Collect appropriate co-payment as indicated by customer’s health plan and refer patient for extra assistance as needed
Package prescription for shipping and assure timely courier pick-up and subsequent delivery
Supervisory
Responsible for oversight of work activities for all pharmacy support personnel and pharmacy interns as required by state and federal regulations
Acts as Supervising Pharmacist when Supervising Pharmacist is not available
Preceptor/Educator
Provide education to patients, providers, and medical staff in written and verbal formats that include formal and informal presentations.
Create opportunities for patient-focused educational workshops and/or pop-up events to educate on chronic disease states and advocate for use of clinic and pharmacy services.
Drug therapy resource to providers and other members of the health care team.
Precept other pharmacy residents and students.

Qualifications
Required:
-Doctor of Pharmacy or BS Pharmacy degree
-Completion of PGY-1 residency or 3 years clinical experience
-Current NYS pharmacist license with immunizer credential
-Current NYS Collaborative Drug Therapy Management credential, or ability to obtain within 3 months of hire (https://www.op.nysed.gov/sites/op/files/prof/pharm/pharmCDTMCredentialsSummary.pdf)
-Minimum of 1 year experience in primary care pharmacy involving direct, collaborative patient care
-Experience precepting students and/or residents
Preferred:
-Current Board of Pharmacy Specialties credential
-Current American Academy of HIV Medicine Pharmacist (AAHIVP) credential
-Certified Diabetes Educator (CDE) or Board Certified-Advanced Diabetes Management (BC-ADM)
-Completion of PGY-2 residency in primary care, ambulatory care or similar experience
-Experience working in community-based health care setting
An understanding of EMRs is helpful. The ability to demonstrate teamwork and effective interpersonal relations skills is imperative. Excellent written/oral communication and computer skills are required.
Physical Requirements
While performing the duties of this job the employee is required to stand, sit, walk, use hands to finger, handle, or feel; reach with hands and arms, talk and hear. Occasionally the employee must stoop, bend and lift or move up to 25 lbs.
