Hourly Pay Range: $32-35 DOE
Pacific Grove Hospital, is a nationally recognized 68 bed acute psychiatric and chemical dependency treatment center offering inpatient & outpatient services for psychiatric illnesses, addictions & co-occurring disorders. PURPOSE STATEMENT:
Plan, direct, define and coordinate individualized and group therapeutic support and services to patient’s withmental health, behavioral health and substance abuse issues.
ESSENTIAL FUNCTIONS:
• Conduct social history assessment of the patient through observation and patient/family interview,obtaining information on the patient’s needs, skills, interests and limitations for the development andimplementation of the treatment plan.• Coordinate and plan programs and activities to meet patient’s social and emotional needs.• Promote recovery, symptom reduction and the achievement of the highest level of social functioning.• Provides crisis management and family sessions to assist the family in understanding the implicationsand complexities of a life situation and its impact on lifestyle.• Assist in the development of effective treatment plans that apply therapeutic principles appropriatelywhile ensuring safety at all times for the patient.• Participate in multidisciplinary treatment team meeting to discuss patient assessment, treatment planand ongoing discharge plan.• Communicate or aid family members in understanding and supporting the patient.• Manage a caseload and assume responsibility for patient chart documentation.• Facilitate education, process groups, support groups and referrals as requested.• Demonstrate a positive, empathetic and professional attitude towards customers always. When patientneeds are not met, acknowledge and work to resolve complaints. Recognize that patient safety is a toppriority.
OTHER FUNCTIONS:• Perform other functions and tasks as assigned.
EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:• Bachelor's degree in Social Work from an accredited college or university, including new graduate required (Masters Degree preferred).• One or more years’ experience working with patient population preferred.
