FlexStaff Residential RN/Nurse Case Manager - Contract - Full-Time in Putnam Valley, New York

FlexStaff is seeking a temporary Nurse Case Manager/Staff Nurse for a contract with our client, a leading organization that provides housing, services and programs for individuals with intellectual and developmental disabilities.

Contract Rate: $80/hr

Schedule: Full-Time - Monday-Friday 9a-5p

Location: Hudson Valley; locations could include Wappinger Falls, Poughkeepsie, Garrison, Yonkers, and Carmel

The Nurse Case Manager will coordinate the delivery of health care services for persons receiving services residing in residential facilities. This position will cover various group home locations in the Hudson Valley.

Responsibilities

  • Coordinates the delivery of routine and specialty health care services for adults with disabilities residing in residential programs

  • Assesses and establishes health care outcomes for persons receiving services. Updates assessments on at least a semi annual basis or as indicated. Maintains health care profiles.

  • Coordinate with the Medical Case Manager Counselors (MCMC) to ensure medical, dental, and specialty appointments, consults, visit documentation, and follow-up appointments are tracked and scheduled according to requirements.

  • Provides guidance and training to direct care staff to ensure proper delivery of health care services.

  • Develops plan of nursing services and oversees the implementation of the health care plan by educating person's receiving services, DSP/Caregivers and families in the plan of nursing services.

  • Evaluates the effectiveness of preventive/ongoing health care and makes recommendations/ referrals for alternative care, as necessary.

  • Acts as a liaison to communicate health information between and among team members, families, other agencies, hospitals, clinics and all other service providers

  • Provide nursing coverage on-site, electronically, or via cell phone between 7:00 am - 7:00 pm, Monday through Friday.

  • Provide complete and up-to-date handover to On-Call nursing service, including emerging and acute situations.

  • Coordinate with the pharmacy to ensure medications have current orders, refills are submitted, and medications are available and administered by AMAP-certified staff as ordered.

  • Review and edit the medication record to ensure the medications are transcribed correctly and monitored as required or assigned.

  • Responsible for maintaining the medication room/ area and monitoring controlled substances as per the regulations/standards.

Requirements

  • NYS RN License, BSN preferred.

  • Prior nursing experience and/or experience working with persons with developmental disabilities preferred.

  • Excellent oral and written communication skills.

  • Able to work flexible hours and/or days.

The salary range for this position is $80-$80/hour
