*Job Summary: * Coordinator will develop, lead, and oversee transition services and work-based learning program for students enrolled in Portage, Schoolcraft of Vicksburg schools.
Essential Duties and Responsibilities:
- TRANSITION:
- Develop and lead trainings on transition planning
- Monitor CIMS/Catamaran, Coordinate/review SPP13
- Support special education teachers in writing compliant IEPs
- Attend monthly County-wide Transition Team (CWTT) meetings
- Collaborate with KRESA transition coordinator to support local districts and students transitioning to post-secondary training programs. (YAP/YCC, Project SEARCH, MCTI)
- Coordinate SSA transition meetings to troubleshoot and provide technical assistance (Team includes: Building department chairs, VCS coordinator/teacher, SCS coordinator/teacher)
- Assist teachers in coordinating visits to KVCC, MCTI, and YAP
- Participate in MTSA/ MCPA
- Collect and report on exiting senior data (Certificates of Completion, etc.)
- Work with Michigan Rehabilitation Services, Michigan Works, and other local agency/ service providers in the Kalamazoo County area
- Work with each building chair to create building-level ownership of transition
- WORK BASED LEARNING
- Work closely with the special education supervisor and director of special education to develop meaningful work sites for high school aged students
- Oversee job coaches at job sites
- Manage student situations that arise on the job sites
- Work closely with district transportation departments to coordinate transportation for all students to and from worksites
- Conduct monthly site visits for WBL with support from teachers
- Conduct regular meetings with job coaches (bi-weekly)
- Maintain electronic documentation of data collected at work sites for case managers to develop progress reports
- Tracks and reports on attendance to businesses and transportation departments
- Maintain Google classroom as a mechanism to communicate with students and building staff
- Monitors students' progress to be able to award credit/ no credit within Skyward grading systems
- Collaborate with special education teachers, guidance counselors, building administrators, and local businesses
- Create, update, and maintain a database to keep relevant information for program evaluation purposes
- Follow-up as needed/requested with students, parents, and school or agency personnel
- Maintain confidentiality of student information and service
- Performs other duties as assigned.

Necessary competencies:
- Knowledge of transition foundations to develop transition education, activities, and services for students, families, and service providers.
- Possession of effective facilitation, coaching, and leadership skills at the individual and group level.
- Ability to engage in collaborative transition service delivery and utilize interagency agreements.
- Ability to facilitate/teach pertinent transition practices (issues) to support special education and agency staff including:
- Understanding of outcome measurement and evaluation of transition services.
