Summary:

  • Pharmacist is responsible for providing comprehensive pharmaceutical care to patients of all ages through integrated drug distribution, clinical services, and provision of drug information. Employee collaborates with medical and nursing staff to ensure high quality, safe medication management. Employee ensures that all medication standards as documented in standard operating procedures and/or state/federal regulations are maintained. The Pharmacist also shares professional responsibility for oversight of Pharmacy Technicians' work to ensure accuracy and timeliness of pharmaceutical preparations. The Pharmacist also shares responsibility for serving as a resource to patients and other healthcare professionals. Employee participates in continual process/quality improvement and demonstrates professional responsibility for ongoing personal development. This position performs decentralized unit – based pharmacy care to patients and other medical professional staff.

Responsibilities:

  • Complies with established corporate and departmental policies, procedures, objectives, quality assurance methods, and safety codes. Demonstrates compliance with licensing, regulatory and accrediting agency.

  • Consistently demonstrates appropriate and professional communication behavior toward patients, customers, and coworkers. Cooperates with management and peers to promote an environment of teamwork and collaboration.

  • Reviews and verifies physician medication orders to ensure appropriate therapy, maintains accurate and completes patient medication profiles.

  • Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, drug-laboratory test interaction, allergies, appropriateness of drug and dose, and therapeutic duplications.

  • Addresses patient own medication requests, obtaining patient medication or coordinating alternative therapy with clinical pharmacy specialists (if possible within 24 to 48 hours of admission).

  • Verify accuracy of all code carts, intubation kits, RRT kits, anesthesia boxes and other emergency kits/boxes.

  • Supervises pharmacy technicians, delegating specific duties and functions as necessary, physically examines and certifies all medications and IVs prepared by technicians prior to dispensing.

  • Detects and reports suspected adverse drug reactions and medication errors accurately and in a timely manner.

  • Compounds and dispenses pharmaceuticals, including sterile, chemotherapy and oral preparations.

  • Issues controlled substances to patient care areas and maintains records as required by law.

  • Maintain annual mandatory educations requirements as mandated by Penn Medicine Princeton Medical Center.

  • Work with the medical staff (providers, nurses, dietitian, respiratory therapist) to manage patient outcomes of assigned areas.

  • Works as an active member of multidisciplinary team and collaborates with healthcare providers within decentralized patient care areas.

  • Consults and communicates with health care professional regarding patients’ drug therapy as required and documents outcomes via interventions (iVents).

  • Participates in the management of medical emergencies (RRT, code blue).

  • Implements, evaluates, and redesigns pharmaceutical care plans based on patient progress and evolving goals for medication usage, including drug dosing, monitoring (ie - nutritional supports, pharmacokinetics monitoring, antibiotic stewardship, and anticoagulation therapy).

  • Perform clinical services including, but not limited to, vancomycin dosing/monitoring, patient medication counseling, antibiotic stewardship, renal function monitoring, IV to PO conversion, discharge medication reconciliation, and admission medication reconciliation.

  • Reviews/interprets culture and susceptibility data for antibiotic appropriateness and recommends changes, as needed.

  • Precept pharmacy students and pharmacy residents.

  • Provides drug information to the medical staff, patients, and their families as needed.

  • Ensures safe, appropriate, cost-effective medication therapies for patients according to established policies, procedures, and protocols.

  • Sustains the Princeton’s formulary by minimizing non-formulary procurements, utilizing therapeutic substitution protocols, and promoting rational drug therapy selection.

  • Participate in process improvement initiatives as needed.

  • Assists in the training and orientation of new Pharmacists/pharmacy residents. Enhances professional growth and development through participation in educational programs, current literature reviews, in-service meetings and workshops.

  • Participates in the development and provides ongoing educational programs, with respective educational departments, for the pharmacy staff, students, nursing staff, medical and other health professionals.
