Responsibilities

The Transplant Quality Specialist works under the direction of the Manager of Transplant Quality to perform duties related to assessing, planning, implementing and evaluating performance improvement and regulatory compliance strategies for SJHMC and Dignity Health Medical Group. In collaboration with the Manager of Transplant Quality and senior leadership, identifies plans and executes activities to promote performance improvement, patient safety and to ensure compliance according to regulatory statutes, policies and procedures.

Responsibilities:

  • Participates in the design and the scope of the Transplant QAPI Program.

  • Demonstrates knowledge of the Transplant QAPI process to include a comprehensive understanding to include all 3 phases of care and services provided by the program.

  • Works with the Manager to develop and maintains the QAPI program for Transplant Services in collaboration with Medical Director(s), the Hospital QAPI Program and transplant leadership.

  • Participates in and assists with the reporting at QAPI meetings related various solid organ transplants. Assist the Manager in maintaining minutes and action planes related to meetings.

  • Works with the QAPI Team to identify quality indicators, appropriate data sources, ensures timely data entry and analyzes reports to facilitate decision-making or QAPI review.Coordinates medical staff and transplant quality team activities, identifies quality issues, summarizes key issues, and facilitates appropriate review through the Quality Management Department.

  • Facilitates identification of quality assurance measures, data collection, reporting and action planning through individuals or appropriate committee or team structures with use of appropriate quality tools and graphical displays.

  • Collects and formats data for various current and future quality dashboards including the indicators being monitored, the data source and the findings, conclusions and actions.

  • Participates in and when called upon can facilitate multi-disciplinary teams to successfully implement quality initiatives and achieve performance goals.

Qualifications

MINIMUM

  • Bachelor's Degree in Nursing or related healthcare field

  • Three (3) years of clinical experience with previous experience in quality performance improvement principles, processes, change Management and TJC required.

  • Strong verbal and written skills. Proficient computer skills required.

PREFERRED

  • Masters degree

  • RN

  • CPHQ

Pay Range

$35.28 - $51.16 /hour
