Description

Position Summary:

The individual in this position acts as the primary consumer advocate, keeps abreast of

progress, and coordinates all services delivered to the consumer and family internally and

through external referral.

Essential Job Functions:

  • Conduct Needs Assessments

  • Develop Service Plans in collaboration with the consumer and family

  • Conduct monthly Service Plan Reviews

  • Document all clinical services and activities in accord with licensing, Medicaid, Joint

    Commission standards, and agency policies and procedures

  • Conduct individual case management sessions, as needed

  • Perform activities on behalf of the consumer and family, including attendance at

    external appointments (e.g., court appearances, etc.), as needed

  • Provide the consumer and family with referrals and linkage to community-based services, as needed

  • Prepare monthly Progress Reports, as required, for assigned consumers for courts,

    probation officers, caseworkers, etc.

  • Coordinate consumer and family care with the treatment team and involved external

    providers

  • Responsible for the development of the discharge plan in coordination with other

    treatment team members

  • Arrange transportation for consumers, as needed

  • Assure implementation of and adherence to the behavioral management system,

    including providing behavioral consequences, motivational incentives, and learning

    experiences for the consumer

  • Participate in Case Conference

  • Provides transportation for consumers as needed.

  • Participate in meetings and attend trainings as required and requested

  • Understands HIPAA, Professional Ethics, Consumer Rights and Confidentiality

  • Participate in performance improvement activities

  • Embrace and embody the mission, vision, guiding principles, clinical vision, and goals of

    WestCare Foundation.

  • Perform any other duties as assigned.

Qualifications

Education:

  • Graduation from an accredited four-year college with a degree in Addiction Counseling,

    Criminal Justice, Social Work, Psychology, or related field is required.

    Experience and Competencies:

  • A minimum two (2) years of related experience providing case management and/or care

    coordination services preferred.

  • Bilingual preferred.

  • Valid driver’s license and ability to obtain and maintain WestCare’s vehicle insurance is

  • required;

  • Must have familiarity of Broward county

  • Must have availability to work a flexible schedule including evenings and weekends.

  • Maintains confidential files and records.

  • Ability to identify and describe key signs, symptoms, impact and manifestations of

    trauma

  • Ability to prioritize and organize competing consumer and family needs

  • Ability to use trauma informed care principles including ensuring consumer safety,

    preventing/avoiding re-traumatization, maximizing trustworthiness, enhancing

    consumer choice, maximizing collaboration, and emphasizing empowerment

  • Aware of the differences between trauma informed and trauma-specific services

  • Demonstrates ability to work effectively staff and populations

  • Knowledge and application of psychopathology theory

  • Knowledge of the inter-relationship between substance abuse and mental health

  • disorders

  • The ability to assess the individual’s growth and development relative to normative

  • development for his/her chronological age, including physical, emotional, cognitive,

  • educational, and social development

  • The ability to conceptualize cases within a developmental framework for the purpose of

    service planning, therapeutic intervention, and discharge planning

  • The ability to coordinate and integrate multiple services, as well as interface and

    communicate with internal and external providers

  • The ability to counsel individuals and families in the achievement of objectives through

    the exploration of a problem and its ramifications; the examination of attitudes and

    feelings; the consideration of alternative solutions; and decision-making

  • The ability to determine eligibility of a consumer’s admission

  • The ability to identify and evaluate an individual’s strengths, barriers, problems, and

    needs for the development of a Service Plan

  • The ability to identify crises when they surface; mitigate or resolve the immediate

  • Problem, and de-escalate the situation, including the ability to develop Safety Plans, as

  • needed

  • The ability to identify the needs of a consumer and family that cannot be met by the

    agency and to assist the consumer/family to utilize the support systems and community

    resources available

  • The ability to provide information to individuals and families concerning alcohol or other

    drug use, mental health problems, and the available services and resources

  • The ability to provide service planning through the identification and prioritization of

    problems needing resolution; the development of goals and objectives for the consumer

    which are expressed in behavioral and measurable terms; and the ability to negotiate

    goals and objectives with the consumer and the family

  • Understanding and application of federal, state, local and agency confidentiality rules

    and professional standards of conduct

  • Understanding of family dynamics, family roles, and family structure

  • Understanding of professional ethics, consumer rights, and professional responsibilities

  • Understands that different cultural and ethnic groups have different perceptions of

    health, wellness, behavioral health, and preventive health services

  • Understands the bidirectional relationships among substance use and mental disorders

    and trauma

  • Understands the differences among various kinds of abuse and trauma, including:

    physical, emotional, and sexual abuse; domestic violence; experiences of war for both

    combat veterans and survivors of war; natural disasters; and community violence

  • Knowledge of WestCare Mission, goals and values

  • Knowledge of Incident Reporting

  • Knowledge of HIPAA, Professional Ethics, Consumer Rights and Confidentiality

  • Familiarity with WestCare services and programs
