Job Description

Job Summary

Responsible for the day to day supervision of practitioners assigned to their region. Works with the Medical Director to ensure practitioners are providing quality of care to patients and access to care is available to all patients.

Knowledge/Skills/Abilities

The Nurse Practitioner Manager will work in collaboration with his/her supervising physicians. He/she will perform comprehensive medical assessments, order appropriate tests/procedures for diagnostic purposes, formulate treatment plans, obtain specialists' consultations as needed, and do appropriate documentations as required.

•Provide clinical education guidelines for Nurse Practitioners, proper HCC coding of clinic and In Home Assessed patients and evaluation of charts of the ACE/Homebound NP's assigned, HEDIS coaching and capture for assigned NP's, evaluating progress notes for completeness and proper coding of the assigned NP's, and acting as a resource for clinical questions and concerns prior to involvement of the medical director.

• May work in a variety of settings (non-clinical or clinical settings) and provide medical care to all levels of patients. Some programs/clinics may focus on specific populations (e.g., adult, geriatric, pediatric).

• Oversee NPs and manage specified programs as required.

• Engage in practices constituting the practice of medicine in collaboration with and under the medical direction and supervision of a licensed physician to the degree required by state laws. He/she may also function as and perform all duties reasonably expected and assigned to an RN such medication administration.

• Responsible for knowing when a patient needs physician oversight and when the patient's medical condition may be beyond the scope of knowledge of an NP.

• Ability to question the patient to ascertain past medical and surgical history as well as a description of any current presenting signs

• Ability to provide telecommunication consultation, including follow-up calls for result reports, etc.

Specific Duties:

STAFF MANAGEMENT

• Provides leadership to practitioners in the cluster, fostering a team approach to deliver medical care and services to patients

• Leads the NP's for the effective delivery of patient care services

• Enforces rules, regulations and policies within the cluster and implement action taken by the company

• Participates in the administration of the NP's through cooperation with the Medical Director in the matters affecting patient care services

• Coordinates In Home Assessment (IHA) assignments and all practitioner schedules

• Reviews Kronos PTO accruals and approve all PTO/CME/CE leaves in accordance with Molina's policies

• Ensure Schedule changes are communicated appropriately to key parties

• Ensure auditing of appropriate mileage claimed prior to approval per policy

• Conducts effective NP meetings and set agenda with Medical director which includes review of policies and procedures, staffing issues, utilization issues, or quality improvement issues.

• Informs practitioners of staffing schedule, productivity and access issues

• Monitors practitioners on time (punctuality) performance and all scheduling changes in accordance with the requirements of the company

• Monitors practitioners Epic In basket to ensure appropriate and timely completion of all In Basket responsibilities

• Coaches NP's where necessary and fosters an educational environment

• Assists in the assessment of NP's

• Keeps patient records complete and up-to-date to assist colleagues in appropriate management of the patient

• Assignment of quarterly peer review at the discretion of the Medical Director

• Participates in the interview of practitioner candidates and the selection of the new practitioners

• Assist in the orientation of new practitioners

• Makes one available to cluster practitioners to keep them informed, helps resolve practitioner problems, and provides feedback on job performance on an ongoing basis

• Assists in the annual performance evaluation of cluster practitioners based on criteria developed by the company

Clinical Duties

• May provide general medical care to various and/or specific patient levels – adults and geriatric.

• Perform comprehensive evaluations including history and physical exams for risk adjustment and other regulatory assessments

• Address both chronic and acute primary care complaints, and able to ascertain medical urgency

• Establish and document reasonable medical diagnoses

• Seek specialty consultation as appropriate

• Order pertinent diagnostic laboratory and radiology testing for the medical diagnosis or presenting symptom; able to work within an environment of limited resources and therefore uses diagnostic tests judiciously and appropriately

• Discuss unusual or unclear symptoms or results with consulting physician in a timely manner

• Accept feedback and direction from the supervising physician

• Create and implements a medical plan of care

• Perform diagnostic and/or therapeutic procedures within his/her level of training and expertise, and as outlined on the practice agreement and written protocol with the supervising physician

• Prescribe medications as appropriate

• Remain knowledgeable on current therapies

• Perform timely documentation in medical records in an electronic medical record computer system.

Job Qualifications

Required Education

• Licensed in the state as an RN and Nurse Practitioner

Required Experience

• At least 6 to 8 years experience as a nurse practitioner

Required License, Certification, Association

• Active and unrestricted Licensed Nurse Practitioner

Preferred Education

Major in gerontology or behavioral health

Preferred Experience

• Bilingual

• Experience or specialization in gerontology or behavioral health.

Preferred License, Certification, Association

Registered in respective state's Medicaid program

Pay Range: $87,568.7 - $189,732.18 / ANNUAL
