Pay Range: $41.12-$67.85*

CHMC-Case Management-Operations Case Management Lead-Full Time-Day shift-8a-4:30-Req 7479

The RN Case Manager – Team Lead provides daily oversight to the case management team to include, but not limited to, guidance related to service delivery and documentation, maintaining compliance with regulatory standards, appropriately managing caseloads, and coordinating schedules. Responsible for training and mentoring colleagues. Likely to own most complex/impactful/risk cases. A RN Case Management Lead coordinates, negotiates, and manages the care of complex patients and families to facilitate achievement of quality and cost effective patient outcomes. This position works collaboratively with interdisciplinary staff internal and external to the organization. Actively participates in performance improvement and evaluation processes related to the management of patient care across the continuum.

Qualifications: Registered Nurse with valid NYS licensure and current registration. Minimum 5 years nursing experience required. Current BLS certification required. Possess strong critical thinking skills and clinical competency. Ability to effectively manage multiple cases. Self-motivated, independent, professional, creative and dependable. Ability to merge clinical and financial processes. Ability to work with others as a team to ensure quality patient care and efficiency. Familiarity with disease management, best practice and current treatment regimens. Ability to interpret Medicare, Medicaid and commercial insurance plans and benefits. Ability to multitask, problem solve complex situations, work around interruptions, and cope with stressful situations. Basic computer knowledge. Maintaining confidentiality of all patient information. Maintaining quality assurance and compliance with all regulatory agencies. Ability to implement and support procedures and programs pertaining to patient safety and security. Exhibit proficient communication skills and ability to deal efficiently and effectively with a variety of personalities. Ability to sustain attention to details and accuracy as it pertains to the monitoring and documentation of information. Able to prepare and present reports. Possess the ability to interpret key data and identify trends.
