WEBER HUMAN SERVICES Certified Case Manager for Therapeutic Community (Weber) in OGDEN, Utah
This position is part of the Therapeutic Community Residential Program located at the Weber County Correctional Facility providing case management services to help clients access services efficiently and decrease gaps between agencies. Job duties include but are not limited to the following: Provide resource education and individual case management services to individuals while they are incarcerated and upon reentry into the community. Services may include a needs assessment, screenings, and community referrals. Provide targeted case management and outreach to individuals with substance use disorders and co-occurring disorders who are moderate to high risk for recidivism. Link individuals to needed resources and coordinate service delivery with the interdisciplinary team as well as with other community providers. Advocate with and on behalf of clients. Work closely with partner agencies, shelters, and landlords to facilitate case coordination and successful transition into securing and maintaining permanent housing. Enter data in various case management systems for tracking and outcomes documentation. Document and manage case plans including shared goal development with a client, monitoring progress, and follow up with case closures. Attend trainings, team meetings, supervision, and any other duties that may be assigned.
Certified Case Manager (must obtain certification through Agency within 1 month of employment). Strong preference for Bachelor's Degree in social work, criminal justice, or related field/or within one year of completing said degree. Further preference for SSW licensure. If SSW licensed, must maintain license by meeting all requirements of licensure including obtaining requisite CEU's. Must have a valid Utah driver's license with good driving record (no more than 1 violation) as this job requires driving.
Must be willing to complete and criminal background check and pre-employment drug screening.
