Home Care Speech Therapist for St. Mary's in Bucks County, PA.

$10K Sign On Bonus

Signing Bonus is not applicable to internal employees, former employees who have resigned in the last year or applicants sourced from outside firms.

Sign On bonus is pro-rated for part time hours

St. Mary Home Care, a member of Trinity Health at Home has a need for a Home care Speech Therapist for lower Bucks County and surrounding areas.

The Speech Therapist consults, evaluates, plans and administers skilled Speech Therapy services prescribed by a physician to homebound patients to restore function, relieve pain and prevent disability following illness, disease, injury, or surgery. Collaborates with Inter-disciplinary team members to assure patient's needs are met, and quality of care is achieved. When acting as the patient care manager, will manage, oversee and provide primary patient care delivery to a select group of patients supporting the patient centered care model, assuring quality and maintaining open communication. Interacts with any and all members of the care group both internal and external to the organization as needed.

Requirements include: 1 year of clinical practice as a licensed Speech Therapist required. Currently licensed to practice as a Speech Therapist in PA. Working knowledge in navigating through Excel and utilization of Microsoft Outlook and Word. Excellent communication and problem solving skills. Excellent organization and time management skills. Ability to multi-task. Excellent inter-personal skills, and ability to work with an Inter-Disciplinary Team approach. Self-confident and assertive, flexible, reliable and dependable. Positive, can do attitude. Currently licensed or eligible for a license to operate an automobile in the applicable state. CPR certification. Regular availability to work. Recent home care experience preferred. Valid Driver's license in PA.
