Today Project FIND operates three supportive housing residences that are home to about 600 people and four senior centers with over 3,000 members. Our members and residents range from the healthy and active to the frail and homebound to the homeless. Job Summary:Committed to providing and coordinating comprehensive supportive services for the clients of our older adult centers. The Social Worker will provide assessment, service coordination, and counseling to older adult clients eligible for receiving case assistance services in the community.General Responsibilities:Provides assessment, services coordination, and counseling to older adult clients eligible for receiving case assistance services in the community. Conduct outreach and intake of older adult center participants at Hamilton and Clinton Older Adult Centers.Manage constituent social work requests related to elected officials and community groups that inquire about social work services for older adults.Screen individuals for benefits, entitlements, and services; assess need and eligibility.Assist older adult clients in applying for benefits, entitlements, and services and follows up to ensure services are obtained; provide supportive counseling as needed.Provide information and referral to other appropriate agencies as needed and identify community resources for consultation and referral.Resolve and/or mediate in potential conflict situations involving center participants.Maintain client case records, work logs, STARS system reporting, and statistical reports.Participate in team trainings, programs, and meetings as assigned.Be available and willing to perform other tasks as assigned by supervisor.Qualifications : This position requires a passionate, hardworking, innovative and hands on person to provide quality services to our clients RequiredBachelor’s in Social Work or Gerontology and two years’ experience in casework or social services.Possess case assistance experience.Be knowledgeable about benefit and entitlement programs for older adults.Ability to work independently and accomplish work responsibilities with minimum supervision.Ability to perform multiple tasks and meet strict timelines.Possesses flexibility, empathy, and compassion in working with the senior population.Excellent problem solving, organizational, time management, verbal, and communication skills.PreferredBilingual in English and Spanish language.Experience working with older adults.Schedule:Monday-Friday from 9am-5pm (35 hours per week). Please note this position requires the availability and flexibility to occasionally work some weekends and holidays.Project FIND participates in E-Verify, which confirms employment authorization through an electronic database maintained by the Social Security Administration and Department of Homeland Security. Minimum Salary: 29 Maximum Salary: 31 Salary Unit: Hourly
