Title: Clinical Coordinator

Program: Clinton Residence

Salary: $59,000-$63,000

Program Description:

Clinton Residence is a 57-bed transitional treatment facility for mentally ill adult men and women with a history of chronic homelessness. The residence provides comprehensive assessments and interventions supporting the recovery model of mental illness. Once supported, to attain their highest level of independence, residents are referred to appropriate levels of permanent housing.

Essential Duties and Responsibilities:

Under the supervision of the Program Director, the Clinical Coordinator will be responsible for supervising clinical staff in counseling, as well as developing, creating, and implementing client centered practices to respond to medical, psychological, as well as any addictions needs; eventually, promoting successful community reintegration for all residents. Other duties and responsibilities include:

  • Coordinates a team of case managers and case aides delivering rehabilitative and restorative services to assigned caseloads of residents.

  • Ensures that consumers are living in safe and clean environments.

  • Functions as a qualified mental health staff.

  • Assures that new residents are oriented to the residence.

  • Performs the function of a case manager as requested.

  • Lead in assessing the needs and strengths of residents and helps the assigned case manager work with residents to set goals and objectives.

  • Ensures that chart documentation is in compliance at all times, review service plans and service plan reviews in a timely fashion.

  • Ensures delivery of services and chart documentation to meet the standards set by the agency and other reviewing, licensing, certifying, monitoring and funding entities.

  • Ensures that at least 2 progress notes are written by case managers each week, that reflect goals and objectives identified in consumers’ service plan.

  • Ensures that clients are linked to all necessary ancillary services including psychiatric, medical and dental treatment, vocational and educational resources, and legal and financial entitlements.

  • Ensures that regular communication with collateral providers occurs and is documented.

  • Provides crisis intervention to ensure the safety of residents and staff.

  • Ensures that post-discharge follow-ups are completed and documented in a timely manner, as indicated.

  • Provides individual supervision to case managers and case aides to help them perform services of training, assistance, support, and related clinical interventions and document supervisory minutes.

  • Prepares performance evaluations and oversees time and attendance records for assigned supervisees.

  • Represents the agency at conferences and meetings of referral sources, treatment providers, and professional and community groups.

    Qualifications:

  • Candidates must have a master’s degree in Social Work or related field.

  • Two years’ experience working with a chronic adult psychiatric population preferred

  • Good writing and communication skills

  • Knowledge of computers

  • Supervisory experience preferred

  • Two years of working experience within the field of Mental Health required

    All newly hired Project Renewal staff must provide verification of being fully vaccinated against Covid-19.
