No On-Call, Weekends, or Major Holidays .
Work Schedule : 10a-6p Central Standard Time
Onboarding : One (1) week in-person orientation at our Nashville, TN Headquarters is required.

About this Role
Belle is seeking a full-time, remote Telephonic Nurse Case Manager to ensure high-quality care, clinical outcomes, and member and caregiver satisfaction.
Our Case Managers advocate for the well-being of our members. As a part of this team, your role will involve reviewing documentation to survey for potential issues and then interacting with members to provide necessary referrals and care coordination to address those issues. Additionally, you will be responsible for mentoring and guiding our Belle Technicians (BTs) in clinical knowledge and skill development, such as data collection, assessment of SDOH, communication with members and caregivers, and documentation of best practices. This will ensure we provide our members with holistic, high-quality care and compassionate support.
Responsibilities
Ensure appropriate referrals are made, track all member cases, and facilitate issue resolution
Utilize excellent communication skills to connect with members, demonstrating empathy towards their situations and clinical circumstances
Seek creative ways to advocate for the member's best interest and remove barriers to care
Provide initial feedback on BT documentation
Liaise with the Operations team to identify trends in documentation challenges
Openly communicate improvement areas to enhance member experience and quality of care
Experience, Skills, & Personality
Registered Nurse
Multi-State Compact Licensure
Minimum 4 years of nursing experience including
2+ years experience with clinical case management
2+ years experience working with the elderly
Strong organizational skills
Naturally empathetic with exemplary communication skills
Self-motivated and directed
Quick and resourceful problem solver
Passionate about population health and working with the elderly
Preferred:
Case management certification
Bilingual Spanish-speaking
Experience with G Suite & Slack
Experience working remotely
