Social Worker
The Opportunity

The Social Worker will work for our Lakeview House, a community residence for adolescents with emotional impairments
who need a structured community setting to result in outcomes of successful functioning and integration back into the family, community, or independent living.
The Social Worker will ensure the effective and efficient delivery of service to clients as defined by OMH as well as the Agency policies and procedures.
Responsibilities
Provide direct counseling and crisis intervention to residents
Create, update and manage treatment plans.
Conduct family engagement events to foster positive relationships with parents and residents.
Complete required paperwork including comprehensive Medicaid billable, case notes, treatment planning, incident reporting and Justice Center Reports.
Implement and develop group curriculum in collaboration with the Program Coordinator.
Communicate with outside providers regarding the overall clinical treatment of residents.
Co-facilitate weekly house meetings.
Responsible for the screening, intakes and enrollment of new residents.
Communicate with outside providers regarding the overall clinical treatment of residents.
Communicate with school districts regarding the educational plans for each resident.
Supervise and guide bi-weekly treatment team meetings.
Provide linkages and referrals to agencies that can assist residents and families with needed services.
Arrange visits with families and prepare the family for reunification.
Identify and schedule workshops for families.
Coordinate discharge planning activities.
Follow discharge youth on aftercare, maintaining at minimum monthly contacts for a period of 3 months post discharge.
Qualifications
Master's Degree required, Licensure preferred.
Two years relevant experience with adolescents or experience within a community residence setting required.
Proficient computer skills with MS Office required.
Valid New York State driver's license with a safe and responsible driving history.
Additional Information
Schedule:
Full Time, 35 hours per week. Some night and weekend hours required.
Hours may vary depending on family and house needs.
Salary: $58,000-60,000/Annually
