Care Coordinator $24.73/hr
We are seeking a Care Coordinator for our Sherpa program, a program that offers guidance to individuals and families navigating the substance use disorder (SUD) treatment system.
Responsibilities:
Identify priority needs and potential barriers to care and priority needs with the goal of eliminating access issues and promote healthy outcomes.
Provide education, risk reduction and case management to individuals regarding substance use, mental health, and physical health services.
Administer screenings for HIV, Hepatitis C, and STI's to at-risk clients and provide linkage to care for all newly infected persons.
Monitor and record participant progress towards improving access through ongoing communication with participants and service providers.
Maintain accurate, up-to-date client files and perform ongoing data entry to internal systems to assist with monthly and annual reporting.
Manage complex care needs and referrals as well as obtain follow-up information on outcomes from providers to ensure continuity of care during the transition between care systems.
Qualifications
High school diploma or GED required; BA degree preferred.
1-3 years of case management experience required.
Experience with substance use disorders a plus.
Must be computer proficient in MS Word and database systems
Valid Driver's license with a safe and responsible driving history

Additional Information
Salary: $24.73/hr
Schedule: (M-F), evening and weekend hours, as needed based on families availability.
This role is full-time (35 hours/week)/ Nonexempt
