Garnet Health Director of Quality and Patient Safety- Physician Practice in Middletown, New York

Responsibilities Under the guidance of the VP Quality/Patient Safety Officer, and Chief Medical Officer, GH Doctors, the Director of Quality and Safety- Physician Practice is responsible for the implementation and development of the GH Medical Group Quality Improvement Program. He/she will be responsible for data management support, education, and facilitation of effective, timely, and integrated group-wide performance improvement activities including, but not limited to: clinical quality care, patient safety and regulatory compliance. Key focus will encompass improvement activities for the primary and specialty-care practices. The role will also collaborate with leaders/ physicians to bridge the acute care and outpatient settings ensuring optimal transitions in care. This position will be a hands-on workflow leader and part of the practice team to implement change management items pertaining to patient care, quality programs and daily clinical flow processes. Director of Quality and Safety- Physician Practice ensures all state, federal and commercial payer quality reporting- pay for performance programs comply with reporting requirements and support optimal reimbursement. This position will serve as an integral part of the training/education process and may be asked to develop, implement and lead the workflow change management process with operational leaders/departments. This position will work directly (through a collaborative partnership) with medical staff and will need to be professional, organized and methodical in the delivery of system changes, quality programs and patient quality safety initiatives. This position will also oversee the RN nurse manager, RN coordinators, care coordination managers and value-based coordinator. Garnet Health System provides a compensation range to comply with the New York State law on Salary Transparency in Job Advertisements. When determining a team member’s compensation and/or rate, several factors may be considered as applicable (e.g., location, specialty, service line, years of relevant experience, education, credentials, negotiated contracts, budget and internal equity) Qualifications Minimum Education: • Bachelor’s Degree in Health Care Administration, Data Analytics, Business or another related field, required. • Master’s Degree, preferred. Minimum Experience: • Minimum of 3-5 years’ clinical experience in an acute care facility, physician practice or other healthcare setting. Quality Management experience preferred. • Experience with Epic and experience in data analytics: collecting, and compiling data from multiple sources for presentation to multiple constituent groups; Need critical thinking skills to provide actionable information and data. • Experience in quality, process improvement, regulatory and/or patient safety in a clinical outpatient area with preference to the medical practice setting preferred. • Possesses outstanding organizational skill with the capability to manage multiple projects simultaneously while maintaining clear focus and direction. • Strong ability to listen, operate at a fast pace, operate at times with ambiguity and be adept at moving quickly to implement change at a rapid pace once decisions have been made. • Superior computer skills including WORD, EXCEL, Power Point, and Practice Management Software preferred. HEDIS, NCQA MIPS/MACRA, pay for performance programs and population health management experience preferred. Required Certification/Registration: • Current and ongoing New York State Registered Nurse license. • Primary Care Medical Home certification required within 2 years of hire or transfer; Certified Professional Healthcare Quality (CPHQ) preferred. Physical Requirements: Travel to multiple facilities is required. Working Conditions: • Environmental Demands and Exposure to Hazards: Works in a clean, well- lighted, heated or ventilated facility. No routine exposure to hazards. • For Remote and Hybrid Remote positions: If deemed operationally necessary, position may be required to report in and work on-site. • Physical Demands: Demonstrates physical and functional ability to perform full anatomical range of motion to accomplish tasks. Evidence of visual and aural acuity and finger and hand dexterity to operate computer and office equipment. Can withstand long periods of sitting, standing and/or constant walking. Ability to lift 10 lbs. • Mental Demands: Ability to foster collaborative relationships, to work well under pressure, to organize and synthesize new information, and prioritize tasks. Possesses critical thinking, analytical skills and flexibility. Ability to multi-task. Required detailed attention to work in an environment where interruptions cannot be controlled. Demonstrates sensitivity to customer needs and expectations.
