PRN Pediatric Licensed Practical Nurse – Rock Fall Area

We’re looking for dedicated Pediatric Nurses (LPNs) in the Rock Falls area —nurses who are more than caregivers; they are advocates, healers, and champions for their patients.

Your Impact as a Pediatric Nurse:

As a LPN , you will play a vital role in delivering high-quality, personalized home care. Your responsibilities include:

• Conducting meaningful assessments, evaluations, and reports to ensure optimal patient care.

• Providing skilled nursing procedures with expertise and judgment

• Observing and effectively communicating any changes in patient condition to the healthcare team.

• Collaborating with families, physicians, and interdisciplinary team members to develop personalized care plans .

• Delivering health education to patients and caregivers to enhance understanding and well-being.

• Accepting and documenting physician orders, ensuring co-signature from an RN Case Manager when required.

• Leading with compassion, understanding, and respect to foster strong relationships with patients and colleagues.

• Maintaining accurate, timely documentation of all care provided.

• Upholding Elara Caring’s mission and values , ensuring the highest quality of care.

What You Need to Succeed:

• Active LPN license in Connecticut (or Compact License).

• Graduate of an accredited nursing school .

• Minimum 1 year of Nursing experience

• Reliable transportation , valid driver’s license , and current auto insurance .

• Ability to travel within the designated area (50%) .

• Physical capability to lift, push, and pull 50-100 lbs .

• A strong commitment to quality, compassionate patient care .
