VNS Health Senior Project Manager in Manhattan, New York

OverviewOversees the integration and implementation of Personal Care data and applications. Supports efforts to increase application adoption in Personal Care and ensures that data is integrated into operational workflows, which includes aggregating of visit duration, tasks performed, care plan adherence, patient location, health status reporting and quality metrics data. Oversees the integration of Personal Care data with the VNS Health warehouse and acts as a liaison between Personal Care, VNS Health, VNS Health Plans, and outside vendors. Works under general direction.

Compensation Range:$98,200.00 - $130,800.00 Annual

• Identifies staffing/resource needed to manage new workflows. Considers organizational impact of proposed process, system and staffing changes. Ensures training, communications and organizational change management activities are included and delivered within project scope. • Tracks implementation and monitors effectiveness of changes, managing/escalating risks and issues, managing budgets, managing vendors, and driving progress through to completion and project close. • Designs daily reporting tools that identify where emergent situations are and where to act quickly to track interventions. • Works jointly with developers, Business Intelligence and Analytics (BIA), and other departments’ to support the planning process, implementation, training and adoption. • Maps current process workflows and assists with solution design of process, system and organizational changes to address business objectives. Develops and applies practical, innovative ideas and approaches. • Develops key metrics to monitor success of processes and adoption. Provides guidance for managing and evaluating reports. • Defines business and functional requirements/specifications for system implementations and improvements. Oversees testing, training, and roll out of system changes. Ensures all users are set up prior to their in-service training. • Participates in special projects and performs other duties as needed. • For Business Intelligence and Analytics Only: • Oversees complex BIA data warehousing projects for Partners in Care. Work closely with Personal Care to build dashboards to support business processes that improve performance. • Articulates BIA issues, information gaps, and data needs; designs project approaches to address needs. Documents business cases for projects and conducts ROI analyses for senior leadership. • Writes and oversees writing of metric documentation, reports, memos, presentations and other project communications, as needed. Delivers presentations in formal and informal settings. • Facilitates interdisciplinary and interdepartmental meetings and workshops with subject matter experts, leaders and project teams. Works collaboratively to ensure stakeholder buy-in to proposed solutions and sustain positive working relationships. • For Capital Grant Only: • Reviews data collection to ensure accuracy. Identifies user errors and follows-up with each employee directly to trouble-shoot. Conducts additional training sessions for Home Health Aides assigned to high risk members and those not using the system correctly or at all. • Identifies reasons for low application adoption (focus groups, surveys, etc.) and develops strategies to increase the adoption. • Assesses patient status reports and revises questions, as appropriate, to ensure accuracy of responses. • Responsible for roll-out efforts of applications to ensure success in adoption of the new technology. • Collaborates with management team to develop clinical protocols and workflows to ensure that all case notifications are triaged appropriately and followed up within an appropriate window of time. Works with the team to assure cases are triaged appropriately during the pilot period. • Incorporates Value Based Payment (VBP) metrics into educational materials and how they can be incorporated into technology. • Develops tiered real-time notification of data to various levels of supervision including the care management team. • Determines potential incentives and internal marketing strategies to encourage employees to use the app. • Participates in special projects and performs other duties as assigned.

QualificationsEducation:

Bachelor's Degree in Business Administration, Healthcare Administration or related field of study required Master's Degree in Public Health, Business Administration, Public Administration, or Healthcare Administration preferred Six Sigma or Project Management Professional Certification preferred

Work Experience:

Minimum of three years of progressive experience in consulting, healthcare, managed care, quality, or a related disciple required Advanced project management and facilitation skills required Strong knowledge of logical business process, i.e., developing and implementing operational processes required Effective oral and written communication and interpersonal skills required Demonstrated ability to lead both small and large teams effectively and be comfortable presenting and communicating to people across the organization required Proficiency in Microsoft Windows Word, PowerPoint, Visio, Excel and Access required For Capital Grants: Minimum of one year progressive experience in consulting, healthcare, managed care, quality, or a related discipline required
