Philips Health Coach - Bilingual in Malvern, Pennsylvania

Health Coach (Malvern, PA or Pleasanton, CA)

Join our Ambulatory Virtual Care (AVC) team as a Health Coach to make a meaningful impact on individuals with chronic health conditions by guiding them towards improved health and well-being.

Your role:

  • Provide telephonic disease management services to patients with diabetes, hypertension, heart failure, and maternal conditions.

  • Analyze glucose reading alerts and conduct a patient assessment to evaluate clinical risk.

  • Provide patient education on topics including nutrition, exercise, stress management, and overall well-being.

  • Foster a supportive and collaborative relationship with patients to facilitate their journey to better health.

Availability/shift requirements:

  • Open availability, inclusive of day, evening, weekend, and holiday schedules is required. Staffing is determined by business needs.

  • Current staffing needs are as follows:

  • 8-hour shifts 8:00am-4:30pm including one late shift 11:30am-8pm per week

  • Friday rotation 9:30am-6:00pm (every 8 weeks)

  • Saturday rotation 8:30am-5:00pm (every 8 weeks)

  • Holiday rotation

You're the right fit if:

  • You have an associate degree and an active CDCES (Certified Diabetes Care and Education Specialist) certification or obtain one within 180 days from the date of hire.

  • You’ve acquired 2+ years of experience in a diabetes related position as a Registered Nurse or Dietician.

  • Your skills include proficiency in both English and Spanish (verbal/written), strong interpersonal skills with the ability to build rapport with patients and thrive in a high-volume, fast-paced environment while maintaining high-quality patient care.

  • You must be able to successfully perform the following minimum Physical, Cognitive and Environmental job requirements with or without accommodation for this position.

  • You’re a team player, excellent communicator, flexible, empathetic, motivate and inspire others, and a good listener with a high level of patience.

Philips Transparency Details

The hourly pay range for this position is $22.00 to $39.00, plus overtime eligible. The actual base pay offered may vary depending on multiple factors including, job-related knowledge/skills, experience, business needs, geographical location, and internal equity.

In addition, other compensation, such as an annual incentive bonus, sales commission or long-term incentives may be offered. Employees are eligible to participate in our comprehensive Philips Total Rewards benefits program, which includes a generous PTO, 401k (up to 7% match), HSA (with company contribution), stock purchase plan, education reimbursement and much more. At Philips, it is not typical for an individual to be hired at or near the top end of the range for their role and compensation decisions are dependent upon the facts and circumstances of each case.
