Responsibilities
The Patient Support Services (PSS) Clinical Social Worker provides individual and group therapy sessions to individuals living with HIV in a multidisciplinary outpatient medical clinic. This role involves providing behavioral health interventions in various evidence-based modalities, conducting assessments, developing intervention plans, and coordinating care to enhance patients' overall well-being and treatment adherence.
Conduct thorough biopsychosocial assessments to identify client needs and develop comprehensive, multidisciplinary PSS Intervention Plans in collaboration with patients and the healthcare team.
Provide individual and group therapy sessions, as well as facilitate support groups, addressing issues such as new diagnosis, disclosure, treatment adherence, mental health concerns, and grief and loss.
Conduct thorough biopsychosocial assessments to identify client needs and develop comprehensive, multidisciplinary PSS Intervention Plans in collaboration with patients and the healthcare team.
Develop and maintain a supportive and therapeutic relationship with clients.
Actively participate in a multidisciplinary care team, communicating effectively with medical providers, case managers, and other healthcare professionals to ensure comprehensive, patient-centered care.
Respond to immediate mental health crises during clinic visits, providing triage, de-escalation, and appropriate referrals to ensure patient safety and continuity of care.
Identify and connect patients with appropriate community resources for mental health services, substance use treatment, housing assistance, transportation, and other social support needs.
Document client progress and treatment plans in a timely and accurate manner.
Qualifications
Experience working with clients who have a history of trauma, substance abuse or mood disorders.
Master’s Degree in social work, counseling, psychology or related field.
Eligible to be licensed as a State Mental Health Professional.

Pay Range
$36.89 - $53.49 /hour
