TogetherTeam is a transformational care delivery model that leverages the intersection of people, process and technology to inform the dissemination of care processes promoting the highest quality and most effective care in alignment with Trinity Health. The Nurse Change Coach supports the Vice President, Change Leadership TogetherTeam in piloting the change management process in developing plans and materials to support and coach the Regional Health Ministries (RHMs), National Health Ministries, System Office Shared Services, and the program team through all change-related aspects and phases of the TogetherTeam implementation, maintaining a common vision, plan, and goal across all areas of the program.

Uses extensive professional experience and knowledge in the development of coaching plans and materials for Regional Health Ministry Executives and local clinical leaders to influence and lead change in the implementation of the TogetherTeam care models. Ensures alignment with Trinity Health enterprise goals in advancing care and continuation in the development of new care models that leverage the virtual platform in additional care areas and encourage personnel essential to the TogetherTeam model to be retained and advance in skill development within Trinity Health and TogetherTeam.

ESSENTIAL FUNCTIONS

  • Knows, understands, incorporates, and demonstrates the Trinity Health Mission, Vision, and Values in leadership behaviors, practices, and decisions.

  • Provides consultative, professional expertise and strategic partnership in leading the design, development and delivery of sustainable culture, change and engagement tools, programs and techniques to support the TogetherTeam human and process change elements within the program structure and processes required to achieve the goals for TogetherTeam.

  • Leads change and manages resistance by leveraging extensive hands-on experience and knowledge to identify barriers and risks to project success.

  • Serves as a key member of the integrated TogetherTeam in the formation and coordination of the change management process and materials (project management, engagement, communication and learning development) and evaluates the impact of change leadership practice on the TogetherTeam effort in each of the Regional Health Ministries.

  • Combines extensive hands-on knowledge of patient care, health informatics concepts, and change management to address the information and knowledge needs of healthcare professionals and patients to promote safe, effective, and efficient use of health IT in clinical settings.

  • Develops action plans, initiatives and communication materials focused on driving executive, clinical and colleague understanding and acceptance of TogetherTeam, to ensure clinical environments and resources are ready for TogetherTeam activation.

  • Develops change implementation plans ensuring all aspects of support including a training program for all users and an internal coach support plan that enables the development of internal expertise while maintaining operational goals.

  • Develops objectives and key metrics to provide regular and concise updates of operational status for the TogetherTeam Core Team weekly and the TogetherTeam Executive Steering Council and other Trinity Health Executive groups as requested.

  • Contributes technical and change leadership expertise in the ongoing refinement and evolution of virtual care models, incorporating Caring Science, continual process improvement, Magnet Standards for nursing excellence, and keeping all desired outcomes in mind. Assists in the development of additional models for new acute care clinical areas, and non-acute service areas.

  • Collaborates in the development and advancement of additional innovative care delivery models that have the potential to be applied to additional specialty areas with acute care and other non-acute care sites. These new innovative care delivery models may incorporate additional disciplines and/or additional technologies.

  • Provides consultative expertise to support and guide Human Resources in the development of retention strategies and programs designed to retain and advance colleagues essential to TogetherTeam operations.

  • Develops and maintains standards of care that inform evidence-based practice, quality of care, patient safety and clinician workflows.

  • Maintains a working knowledge of applicable Federal, State and local laws/regulations; the Trinity Health Integrity and Compliance Program and Code of Conduct; as well as other policies and procedures in order to ensure adherence in a manner that reflects honest, ethical and professional behavior.

MINIMUM QUALIFICATIONS

  • Master’s degree in Nursing, Business Administration, Health Informatics, Healthcare Administration or related field, or equivalent combination of education and experience.

  • Current Registered Nurse licensure.

  • Coaching certification beneficial.

  • Prefer, Change Coach with experience implementing patient care delivery model.

  • At least ten (10) years of progressively more responsible experience including senior level leadership experience with health care operations and program management at the enterprise and/or national level.

  • In-depth knowledge of change management methodologies and experience in driving efforts to develop and apply transition and sustainability plans.

  • Ability to operate in an ambiguous and highly matrix organizational structure. Ability to operate in a highly autonomous self-directed manner under frequently changing structures and requirements and work priorities.

  • Strong consulting and execution skills. Ability to define program or process actions to achieve department strategy, goals and performance measures.

  • Strong interpersonal, communication, consultative, relationship building and conflict management skills in order to initiate and develop productive, collaborative partnerships with all levels of service partners and leadership across the organization. Ability to foster relationships built on sustainable trust in order to provide long-lasting, comprehensive advice and service. Ability to effectively influence results, garner support, and tactfully manage complex relationships within and across the organization. Extremely high level of diplomacy and tact are required. Ability to read the subtle nuance of situation and react/plan accordingly.

  • Proven ability to manage projects independently and effectively coach/mentor.

  • Ability to be a front-line change agent to enable and deliver long-lasting and sustainable organizational changes while minimizing disruption to the patients, associates, and business. Seeks innovative ways to deliver higher value programs and services more efficiently and effectively.

  • Analytical ability to resolve complex issues and conceptualize well in a fast-paced, dynamic and multi-functional environment requiring creativity, innovation, foresight and mature professional judgment. Serves in advisory/consultative lead role. Ability to operate in a collaborative, shared leadership environment and has wide latitude for independent action.

  • Must possess a personal presence that is characterized by a sense of honesty, integrity, and caring with the ability to inspire and motivate others to promote the philosophy, mission, vision, goals, and values of Trinity Health.

PHYSICAL AND MENTAL REQUIREMENTS AND WORKING CONDITIONS

  • Works in a normal office environment, requiring the ability to frequently respond to unpredictable situations.

  • Must have the flexibility to work with a multitude of customers and complex issues.

  • Must be able to travel to the various Trinity Health sites with considerable travel requirements up to 30-40 % of the time.

  • Must be able to work and think independently. Will be required to be creative and innovative when working both independently and in large and small group settings. Work also performed in meetings (formal and informal with colleague and non-colleague attendees).

  • Working hours may be long and irregular.

  • Occasionally exposed to external elements.

  • Must be able to speak and communicate clearly and effectively.
