Androscoggin Home Healthcare + Hospice Community Care Team Clinician - 40 Hours in Lewiston, Maine
Title: Community Care Team Clinician, Cumberland & York County coverage.
What You’ll Do:
Collaborate with and coordinate community networks/teams and community-based support and service providers who share resources to ensure patients can access all the available supports the community has to offer.
Establish a supportive and connected community to improve the health of the patient, reduce unnecessary re-hospitalization and impact on health care services. CCT will support this goal by collaborating with the patient the PCMH, and community partners to assure that identified needs are addressed.
What You’ll Bring:
Current RN, LPN, LSX, LSW, LMSW-CC, LMSW or LCSW license in the State of Maine.
Current CPR card required.
One year of recent clinical experience required.
A reliable car with insurance.
