Summary:

  • Under Pharmacist supervision, the Pharmacy Technician Care Coordinator communicates with nurses, physicians, pharmacists, and other medical team members to assist patients with medication access and adherence. Roles may include investigation of pharmacy benefits, patient adherence evaluation, copay assistance with manufacturer programs, coordination of recurring medication refills, and delivery services for select medication access channels. Effective communication, critical thinking, and organizational skills are necessary for success in this position. The pharmacy technician care coordinator communicates and collaborates with supervisors, pharmacists, peers, patients/customers, and outside agencies as necessary. The role of individuals in this position facilitates care across multiple sites and transitions in care. The successful candidate will be conscientious and focused on contributing to high-quality patient care.

Responsibilities:

  • Communicates and documents appropriately and effectively with supervisors, physicians, nurse practitioners, clinical pharmacists, social workers, nurse managers, peers, patients/customers, and outside agencies.

  • Collaborates with internal team members to gather appropriate paperwork, prescription, billing information, and/or prescription(s).

  • Coordinates medication access and delivery, as applicable, to ensure timely treatment.

  • Develops and maintains an understanding of medication names and drug therapies.

  • To ensure ongoing medication access, provides appropriate follow-up relating to patients, clinical practices, and family for pharmaceutical needs and/or additional eligibility information.

Credentials:

  • CPhT- Certified Pharmacy Tech (Required)

Education or Equivalent Experience:

  • H.S. Diploma/GED (Required)

  • 1+ years pharmacy experience as a certified pharmacy technician in a retail/outpatient or infusion pharmacy, or payor environment

  • Other

  • 2+ years pharmacy experience as a certified pharmacy technician in a retail/outpatient or infusion pharmacy or payor environment
