Work Schedule: Days Evenings Nights with Weekend and Holiday Rotation
Hours: Primarily 7am - 3pm with occasional flex to 2pm-10pm and overnight shifts as needed. Weekend and holiday coverage are also rotating shifts (day/evening mostly, some nights).
Summary:
Works with the System Director of Pharmacy Services, system, and site pharmacy leadership in the establishment and maintenance of a comprehensive Clinical Pharmacy Services Program specializing in direct patient care areas such as Emergency Department, Critical Care, Infectious Diseases, Psychiatric Pharmacy, Ambulatory Care, Oncology, Internal Medicine, and select areas of specialization where pharmacy residency training is desirable. Areas of Critical Care specialization include all of the various areas of focus in Catholic Health including neuro, cardiac, medical, surgical, and neonatal. Incumbent coordinates and actively participates in various programs and activities within their area of specialty and within the scope of the Clinical Pharmacy Services Program. Provides educational programs for nurses, pharmacists, medical and pharmacy residents, physicians, mid-level providers, patients and their families. Precepts pharmacy residents across the system. Assists System Director of Pharmacy, system, and site pharmacy leadership in maintenance of readiness by regulatory agencies and in ongoing compliance with all statutory regulations.
Responsibilities:
EDUCATION AND EXPERIENCE
Doctor of Pharmacy degree (PharmD) with PGY1 Pharmacy Residency in an acute care or ambulatory care setting required; and a PGY2 Pharmacy Residency in area of specialization preferred. Current residents accepted for application. Board Certification preferred, and required within one year of hire or
PharmD with completion of a PGY-1 Pharmacy Residency in an acute care hospital setting. Board Certification preferred, and required within one year of hire. One year of fulltime (or equivalent) experience in area of specialty preferred
Residency requirements
All Pharmacy Residencies require American Society of Health-System Pharmacists (ASHP) accreditation or candidate status
All residencies must be verified for completion through presentation of certificate of completion from residency site and/or the database of the ASHP
Candidates currently enrolled in an accredited or candidate status pharmacy residency program are eligible to apply. Evidence of residency completion must be available prior to start date
Experience Requirements
Relevant past experience must have been obtained in entirety within three years of application
Required full time equivalent experience may be accrued over three year period
Board Certification
Must be obtained through nationally recognized organizations such as the Board of Pharmacy Specialties (BPS)
Board Certifications preferred at the time of hire must be in a clinical area of specialty relevant to inpatient hospital care
Specialty specific board certification (e.g. Emergency Medicine, ID, Critical Care) must be obtained within one year of eligibility and/or availability of certification in area of specialty
Ongoing maintenance of board certification required
Preceptor Requirement
Within 12 months of hire, maintains compliance with preceptor qualifications set forth by the American Society of Health-System Pharmacists (ASHP)
Licensure and Academic requirements
PharmD degree must be obtained from an ACPE accredited US school of pharmacy or a foreign school recognized by BPS and/or ASHP
Valid license and current registration to practice Pharmacy in New York State
KNOWLEDGE, SKILL AND ABILITY
Ability to analyze/interpret and utilize results of various laboratory and microbiology tests and apply them to patient care in a clinical practice setting
Excellent interpersonal skills and the ability to convey educational material in an understandable manner to patients and their families.
Ability to coordinate activities of multiple personnel and supervise their daily activities
Ability to use hospital based information systems including Pharmacy, Laboratory, Patient Accounting and office systems
Ability to operate ancillary patient care information systems in conjunction with operation of patient testing equipment
Ability to communicate effectively with Physicians, Physicians Assistants, Nurse Practitioners, Nurses and other allied health professionals
WORKING CONDITIONS:
Travel to various locations for clinical assignments, meetings, and educational programs required
Works unusual hours, days and shifts to accommodate needs of the educational activities of the Clinical Pharmacy Services Program
Works in a clinic setting with considerable periods of standing and sitting
